Description
This is a combined synopsis/solicitation. Request for Quotation (RFQ= ) 618-36-03 entitled "Syringe Sets" is issued under Statutory Authority 41 U.S.C. 253(c)(1) and IAW FAR 6.302 and VAAR 811, the proposed contract is for Brand Name or Equal supplies and is hereby issued for commercial items to be used at the VA medical center and is prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and incorporates provisions and clauses in effect through FAC 010-08.The authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by Brand Name or Equal supplies. The NAIC code for this procurement is 339112 and the small business size standard is 500 employees or less. This BOS announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. Contractor's proposals are hereby being requested in response to requirements for brand name or equal products listed below and shall be submitted on contractor's letterhead or bid form and must address all requirements listed and must contain the name, title, and signature of person authorized to submit proposals/bids on behalf of the contractor. VA Medical Center, Minneapolis, MN has a requirement for brand name or equal Merit Medical Systems, Inc., South Jordan, UT. Item One =96 1020 each - Polycarbonate Syringe Set, stock number K02-00362, disposable, sterile, color coded set in peel back pouch consisting of: three (3) each - 30cc polycarbonate syringe, luer lock, clear barrel labeled Saline with blue color plunger; two (2) each - 10cc polycarbonate syringe, luer lock, clear barrel, labeled Xylocaine with red color plunger; two (2) each - 10cc polycarbonate syringe, luer lock, clear barrel, labeled Contrast with yellow color plunger; one (1) each - 3cc polycarbonate syringe, luer lock, clear barrel, white color plunger. This is a requirements contract for Fiscal Year 2003, with two 12-month options to renew for Fiscal Year 2004 and 2005. Price proposal will include separate price schedules for each Fiscal Year. The VA is requiring FOB destination pricing. All supplies provided will be Y2K compliant. All responsible sources who can meet the VAMC's requirements and provide items as listed above may respond to this solicitation by submitting a non-sterile bid sample along with price and technical proposals on their company letterhead or bid form. Evaluation factors will include, but may not be limited to, delivery, evaluation of bid sample, past performance and price/price related factors. As evaluations may be subjective in nature, VA staff are solely responsible in determining acceptability of equal products. Products will be delivered either to the VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417 or to the Medical/Surgical Product Prime Vendor. The solicitation clauses incorporated herein are those in effect through FAC 01-08 and are applicable to this solicitation: 52.212-1 Instructions to Offerors; 52.212-2 Evaluation Commercial Items; 52.212-3 Offeror Representations and Certification-Commercial Items (NOTE Offerors must include a completed copy of Offeror Representations and Certification along with his/her offer); 52.212-4 Contract Terms and Conditions Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes on Executive Orders Commercial Items; 52.217-6 Option for Increased Quantity; 52.217-9 Option to Extend the Term of the Contract; 852.219-70 Veteran-Owned Small Business; 52.211-6 Brand Name or Equal; 52.216-21 Requirements; 52.216-18 Ordering; 52.216-19 Order Limitations; 852.211-77 Brand Name or Equal, and Y2K Warranty Clause.
