- NA Western Regional Contracting Office intends to procure 1500 each medical for Madigan Army Medical Center. The RFQ DADA13-02-T-0090 will be available upon request to Mitch Munger via e-mail, at mitch.munger@nw.amedd.army.mil on or about 16 September 2002 w ith a closing date of 25 September 2002. Vendors requesting a copy of this RFQ must provide Business Size, telephone number / facsimile number, e-mail address and a point of contact. This RFQ is being issued on a Small Business Set-Aside basis using Simpl ified Acquisition Procedures (SAP). Per DFARS 252.204-7004, Central Contractor Registration is mandatory for all contractors in order to receive award of a contract or order. The NAIC Code is 314911 with a business size of 500 employees. ****
