- This procurement action is for the Naval Medical Center, San Diego, CA, 92134. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and no written solicitation will be issued. The solicitation number, N00259-02-T-0120, is issued for reference purposes. Closing date for this solicitation is 19 Sep 2002. All inquiries and/or quotations should be directed to the purchasing agent, Kohala Hembre, e-mail: kohembre@nmcsd.med.navy.mil, or fax 619-532-5596. No phone quotations will be accepted. Award will be made based on technical ability, past performance, and price. These evaluation factors have equal levels of importance. For past performance evaluation, provide at least three (3) references of sources for whom you have provided the specified materials and/or service(s). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-09. STATEMENT OF WORK: All reagents requested on this solicitation must be delivered with a minimum of six (6) months shelf life. Delivery for all line items is to be to Lab Supply, Room 3D-20E1, attention Complex Testing with the following delivery schedule: 03 Oct 02, 03 Jan 03, 03 Apr 03, and 03 Jul 03. MSDS to be included with each delivery. CLIN 0001: Enzyme-linked immunosorbant assay (ELISA) used for the qualitative detection of IgG antibodies to Varicella Zoster in human serum. 96 tests/kit. Name: Varicella Zoster IgG ELISA; Product #: 425600; Delivery Schedule: 11 kits/delivery. Qty: 44 kits. CLIN 0002: Enzyme-linked immunosorbant assay (ELISA) used for the qualitative detection of IgG antibodies to Herpes Virus in human serum. 96/tests/kit. Name: Herpes Group IgG ELISA; Product #: 446600; Delivery Schedule: 4 kits/delivery. Qty: 16 kits. CLIN 0003: Enzyme-linked immunosorbant assay (ELISA) used for the qualitative detection of IgG antibodies to Heliobacter pylori in human serum. 96/tests/kit. Name: H.Pylori IgG ELISA; Product #: 446400; Delivery Schedule: 5 kits/delivery. Qty: 20 kits. CLIN 0004: Enzyme-linked immunosorbant assay (ELISA) used for the qualitative detection of IgG antibodies to Mumps in human serum. 96 test/kit. Name: Mumps IgG ELISA; Product #: 425900; Delivery Schedule: 6 kits/delivery. Qty: 24 kits. CLIN 0005: Enzyme-linked immunosorbant assay (ELISA) used for the qualitative detection of IgG antibodies to Measles in human serum. 96 tests/kit. Name: Measles IgG ELISA; Product #: 426000; Delivery Schedule: 8 kits/ delivery. Qty: 32 kits.
