- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. F13C9022331400 is being issued as a Request for Quotation (RFQ), which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2001-03. This acquisition is reserved for small businesses; however, the Government is accepting proposals from large businesses. In the event of insufficient small business participation, the award will be made to a participating large business. The associated NAICS code for this acquisition is 42145. McConnell AFB, Kansas, plans to procure the following items: Line Item 0001: Senographe DMR+ mammography unit which includes the following: telescopic column, high-frequency generator, control console, x-ray protection screen, face protection shield, 18 x 24 SharpIQ Bucky, 18 x 24 cassette older, 18 x 24 compression paddle, 24 x 30 SharpIQ Bucky, 24 x 30 compression paddles, auxiliary paddle, square spot compression paddle, magnification plate, user manual, dual foot switch ? compression & vertical column height, dataflash, small round spot compression paddle, 9 bar markers, bar marker install kit. Discount provided by company for trade-in of old DMR, valued at $15,000.00. PN S3032J Mfg: International General Electric Quantity: 1 each Line Item 0002: Set of eight mammography suction markers. PN S30311LM Mfg: International General Electric Quantity: 1 each Line Item 0003: 2D biopsy paddle with metal crosshairs for DMR+. PN S3032JA Mfg: International General Electric Quantity: 1 each Line Item 0004: Senographe Accessory Cart, 35 inches long x 20 inches wide x 34 inches High, designed to hold compression paddles, two Buckys, cassette Tunnel, magnification platform, film marking device, sterotactic system, two supply drawers. PN S30311HL Mfg: International General Electric Quantity: 1 each Line Item 0005: MQSA compliance guide. PN S30311AZ Mfg: International General Electric Quantity: 1 each Line Item 0006: 2 day on site visit to train up to four technologists on the equipment use, system theory and parameter selection for patients. PN W0100MM Mfg: International General Electric Quantity: 1 each Line Item 0007: DE-install and removal of old DMR+ system, shipping and installation of new DMR+ system. Quantity: 1 each The following provisions in their latest editions apply to this solicitation: FAR 52.212-1, Instructions to Offerors-Commercial Items; FAR 52.212-2, Evaluation- Commercial Items (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factor shall be used to evaluate offers: Price; FAR 52.212-3, Offerors Representations and Certifications- Commercial Items (offerors shall include a completed copy as part of their quote); FAR 52.212-4 Contract Terms and Conditions- Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items (Deviation), FAR 52.219-6, Small Business Set-Aside, DFARS 252.204-7004, Required Central Contractor Registration; DFARS 252.212-7001 (Deviation), Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. Additional FAR clauses cited within the above clauses, which are applicable, will be incorporated into the solicitation and resulting contract. Award will be made to the most responsive offeror proposing the Senographe DMR+ mammography unit determined to be the most advantageous to the Government in accordance with the evaluation factors listed above.
