- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.(II) Solicitation F7MWR122240200 is being issued as a request for quotation (RFQ). (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 01-07. (IV) This requirement is being synopsized as a 100% small business set-aside under standard industrial classification code 1752, NAICS Code 235520, small business size standard of 12.0 Million. (V) Please quote the items as listed: Line item 0001, Veplas Tiles - Interlocking dowels - 2 feet x 2 feet x 2.25 inches - ASTM tested with the rating of 6 foot 3 inches fall height - Cover approximately 1000 square feet. Line item 0002, Wedge Tiles for perimeter - 2 feet x 2 feet x 2.25 inches - ADA Compliant - Cover approximately 300 square feet. Line item 0003, Non-Personal Service - Contractor is to provide all labor, equipment, personnel and materials for removal and disposal of old fall zone, approximately 1300 square feet. - Clean concrete slab to prepare for new surfacing and install new surfacing. (VI) All labor and materials must comply with CPSC Guidelines for playground safety. (VIII) Provision 52.212-1, Instructions to Offerors- Commercial Items applies. (IX) Provision 52-212-2, Evaluation _ Commercial Items applies. Quotation will be evaluated on price and past performance. The evaluation factor of past performance will be approximately equal to cost or price. (X) Offerors are required to submit a completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items with their offer. (XI) Clause 52.212-4, Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to the award document. (XIII) Subject to the AVAILABILTY OF FUNDS. Funds are not currently available for this project. Award may be made without notification once quotes are received and funds are secured. (XIV) N/A. (XV) N/A. (XVI) A site visit will not be held. Quotations will be due to the 36th Contracting Squadron, Unit 14040, APO, AP 96543-4040, by 23 Sep 2002, 3 p.m. local Guam time.
