- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. 613-69-02 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-09. This acquisition is a 100% SMALL BUSINESS SET-ASIDE under the NAICS code 562119, and small business size standard of $10.5 million per annum. The VA Medical Center, Martinsburg, WV intends to contract for removal and disposal of lime according to Federal EPA and State DEP regulations from two (2) lime beds at the Water Plant. Lime beds are approximately 50' x 100' x 4". After removal of lime, the Contractor shall add a minimum of 6" of ballast stone, as needed, to the beds for proper drainage. Contractor is responsible for all licenses, fees, etc. that apply. Contractor shall be required to coordinate with the Contracting Officer's Technical Representative (COTR) at least five (5) working days in advance of performing work. All work shall completed within thirty (30) calendar days after contract award. The following ten (10) days are observed by the Federal Government and no work shall be performed on these days: New Years Day, Martin Luther King Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day designated by the President of the United States to be a national holiday. One lump sum payment shall be made monthly, thirty (30) days in arrears, upon receipt of a proper invoice. Invoices shall be submitted in accordance with FAR 52.212-4(g) and shall be submitted to Chief, Fiscal Section (613/BPO-04), VA Medical Center, Route 9, Martinsburg, WV 25401. The provisions at FAR 212-1, Instructions to Offerors-Commercial Items applies to this acquisition. Offers shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition, to include the following addendum to the clause: 52.228-5, 52.232-18, 52.233-2, 52.233-3, VAAR 852.233-70, 852.233-71, 852.237-70, 852.270-1 and 852.270-4. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial items, applies to this acquisition which shall include the following clauses: 52-222-21, 52-222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.232-34 and 52.222-41. Offerors shall submit their quotations along with a completed copy of FAR 52.212-3, Offerors Representations and Certifications-Commercial Items to the Contracting Officer, VA Medical Center, Route 9, Martinsburg, WV 25401. Faxed offers will be accepted (FAX #304-264-3997). The Service Contract Act applies to this acquisition, which provides the minimum wages required by law to be paid to service employees, performing services under this contract. The Wage Determination in effect for this period is Wage Determination #94-3017, Rev. 2, dated 6/5/02. A copy of this wage determination may be obtained from the Contracting Officer.
