NA STATEMENT OF WORK CATHOLIC PRIEST The Contractor shall serve as Catholic Priest for the Fort Buchanan Catholic population in accordance with the Army Regulation 165-1, Fort Buchanan standard Operating Procedures and the Archdiocese for the Military Services. He will conduct services per t he dates of the Roman Catholic Church and as outlined in this statement of work. He shall also provide additional pastoral ministrations in counseling, hospital visitation, Bible studies and other lay participation activities that support worship services . All coordination giving and receiving official documentation, emergency ministrations, Military Council of Catholic Women, and administration related to parish activities shall be incorporated in end products described in this statement of work. As a n on-personal service contractor, the Contractor is not a member of the Installation Chaplain Office Staff. The Contractor must be able to speak, write (to understand and be understood) English and Spanish. All ministrations will be performed in English an d Spanish when required and planned. Definition of Sunday Sabbath/Holy Day Worship Service. A Celebration of the Eucharist (Mass) utilizing the liturgy of the missal following the church calendar for a congregation. The priest, in all Catholic Worship Services, shall include spiritual prepa ration and sermon preparation. The Mass is held on Saturday afternoons (1730-1830), Sunday mornings (1000-1100), and designated Holy Days at time to be determined. Weekday Service. A Mass or para-liturgical service will be held during weekdays, Monday through Friday, unless the weekday happens to be a Federal Holiday, in which case there will be no Mass, it must be announced at least two Sundays prior. Sacraments. A formal Catholic rite. The Roman Catholic Church celebrates seven sacraments to include: Eucharist, Baptism, Reconciliation, Marriage, Confirmation, Anointing of the sick, and Holy Orders. The Contractor shall provide for these entire Sacr ament, except Ordination. Location. Primary place for the conduct of services will be the Post Chapel, Fort Buchanan, building 183. The contractor, upon assignment by Mass schedule and/or by the Installation Chaplain shall conduct worship services or other religious activities in designated chapel or facilities in Puerto Rico. Workmanship. All work shall be accomplished within the guidelines of the Roman Catholic Church, Army Regulation 165-1, Command Regulations, directives, and the Installation Chaplain?s Policy statements. The contractor shall be liable for loss, damage or theft of any Government property due to the contractor?s carelessness and /or negligence. Negligence will be determined by appropriate Report of Survey findings as determined by the Command. All services under this contract are performed by an ordained member of the Roman Catholic Church and endorsed by the Archdiocese for the Military Services. The contractor shall be experienced and fully qualified to perform the required services with litt le or any supervision or direction by Government personnel. Contractor Points of Contact. The contractor shall provide the Contracting officer, and the Contracting officer?s Representative (COR), with names of a primary and alternate point of contact with address, e-mails, and telephone numbers, in case of emergen cy. Personal Property. The contractor shall ensure that personal property is clearly marked and identified to preclude misidentification as Government property. For example, robes stole communion cup, and/or other ecclesiastical equipment that could be simil ar to and identified as Government property. Performance Evaluation Meetings. The contractor shall meet with the Installation Chaplain once per month, or when required by either party. The contractor shall contact the Installation Chaplain, to make appointments for the meeting. The purpose of these meetings will be to discuss the ne eds assessment of the Catholic population located at, or supported by Fort Buchanan, program evaluation and program/activity scheduling. The contractor and/or the COR shall notify the Contracting Officer for discrepancies, disputes or indications of poor performance. The Installation Chaplain, if not the COR, shall notify the COR of such indications. Hours of Operation Performance. The contractor shall provide the required products and services as indicated in the delivery schedule of the contract: Ministrations and Emergency Services. Emergency services required for Roman Catholic Emergency ministrations only. The contractor shall receive payment for those specific emergency ministrations required, and as specified, in this contract. The contract or will provide emergency ministrations to include, but not limited to, anointing of the sick, Baptisms, Eucharistic and Confessions. Government-Furnished Property and Utilities. Office and Equipment. The Government is not obligated to furnish office space, automation equipment or office supplies, permanent or provided, in the Chaplain?s Office, Fort Buchanan, Puerto Rico. However, when ever possible, appropriated office space, f urnishings and equipment for the support of the contract and the performance of the products and services, could be provided. Office space, office equipment, and office supplies are not permanent or dedicated solely to the use by the contractor. Liturgical items. The Government will provide all required consecrated and liturgical items of equipments and supplies needed in the conduct of a Roman Catholic worship Services, providing of the Sacraments, and other Ministrations. Utilities. The Government will provide all utilities, such as electrical and water, to perform the services required by the contract, at site of delivery of the services on federal property. The Government will not make any modifications to utility outle ts, water faucets, etc., to accommodate contract equipment. The Government will not furnish the contractor with Government living quarters. Contractor Furnished Property and Utilities. The contractor will furnish his own transportation to and from the Fort Buchanan Chapel/Chaplain office or place of worship/ministration, to include any location requiring emergency Catholic coverage. The contractor will be furnished, when ever available and possible, the use of a typewriter and/or word processor for the presentation of lessons, sermons, plans, handout, forms, statistical data to be submitted as required by Church Headquarters, or to t he Installation Chaplain. The contractor may use personal ecclesiastical equipment to conduct Roman Catholic Worship Services, Sacraments and Ministrations. The Government will not be liable for loss, damage, theft or maintenance of personal ecclesiastical equipments. The contractor will not be required to furnish any office, ecclesiastical or liturgical supplies in order to accomplish the requirements of this contract except as specifically noted. Work to be accomplished: The performance of all ministerial duties will include Services, Sunday, Saturday and Holy Day Masses, Weekday worship services or Masses, Sacraments of Baptim, Matrimony, Reconcilliation, Confirmation, Memorial Services, and Anointing of the sick, religio us education, and emergency coverage. Ministrations. The contractor shall provide, as often as necessary, and requested by the Installation Chaplain, assistance and instruction to the Adult Education Program, to include lector and Extraordinary Minister of the Eucharist Training. Shall attend and/or conduct weekly Bible Study Sessions for the Catholic Community. Shall provide counseling sessions to Catholic? members, authorized military, DOD, and authorized Government personnel and dependents upon request. Shall visit the hospitalized/or auspiciod members, or any military, DOD, or authorized Government personnel depend ents who request to see a priest, providing the Sacrament of Anointing of the sick and/or Communion upon request. Shall serve as Post Chapel Catholic Duty Chaplain in Emergencies only. Shall conduct pre-marriage counseling. Shall attend all Catholic Parish Council Meetings. Commander?s Master Religious Program (CMRP). The Installation Chaplain will incorporate the contract statement of work (SOW) into the CMRP. Chaplaincy Program Budget Advisory Committee (CPBAC). The contract will not serve as a member of the CPBAC, but will be represented by the Installation Chaplain. The representative shall present the needs of the Catholic Program. The contractor may atte nd the meetings of the CPBAC as a visitor and may provide information as required. The contractor will not participate in any decision making or voting. Administration Administration for the purpose of this contract is the completion of report, Government form, and other input that may be required for each contract product or service. Documentation or Work/Reports. The contractor will maintain a daily log of work performed under this contract, and will provide a copy of the log to the Installation Chaplain each month, not later than close of business and on the 5th calendar day of the following month. The contractor will document worked performed on the invoice, in order to request payment for contracted services. Request for Supplies/Equipment. The contractor will provide the Installation Chaplain, 60 calendar days prior, a request for supplies and equipment for supporting the Catholic Program, in accordance with Chaplain?s Fund SOP and Chaplain Resource Manager I nstructions (For appropriated, and non-appropriated Fund Supplies). The Chaplains? Fund Manager and the Chaplain Resource Manager will provide applicable forms, SOPs, and instructions to the contractor. Privacy Act. The contractor shall be familiar with, and insure compliance with, provisions of the Privacy Act, Freedom of Information Act, and limitations on Government collection of personal statistical data on non-affiliated personnel. Applicable Government regulations concerning these Acts will be available at the Installation Chaplain Office for review. *The contractor will personally perform the services described in the statement of work or will cause them to be performed by a substitute of equal or higher proficiency, and shall notify the Installation Chaplain in advance. Will be the sole responsibili ty of the contractor to pay the substitute. *This contract will be terminated with in 30 days of an Active Duty Catholic Chaplain reporting for duty, or when determined by the Command. (SEE ATTACHED AR 165-1, 13-4) Priority of Work The contractor will give priority of work to life and death situations that require pastoral response.
