Description
This is a combined synopsis/solicitation, for commercial items prepared in accordance with the format of FAR 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation Number N68096-02-Q-0216 is hereby issued as a Request for Quote (RFQ), and is solicited using 100% Total Small Business Set Aside. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The North American Industry Classification System (NAICS) Code is 561621. The U.S. Naval Hospital, Guam has a requirement for a SECURITY SURVEILLANCE CAMERA SYSTEMS, BRAND NAME OR EQUAL, if offer is equal FAR Part 52.211-6 applies. The list of Contract Line Item Numbers are as follows: Spectra 111 color cyber dome with controller, quantity 1 each, Low light color cameras in exterior grade housings, quantity 4 each, IR illuminated color camera in an exterior grade housing, quantity 1 each, Color dome cameras with varifocal lens, quantity 24 each, 21? color monitors, quantity 2 each, 14? color monitors, quantity 2 each, Sixteen camera multiplexers, quantity 2 each, Digital vault with built-in multiplexer and 320 GIG hard drive, quantity 1 each. Also included in the contract: All UTP and fiber conversion and termination products, all wire and labor necessary to complete the installation, with a full one year warranty on parts and labor. Warranty excludes acts of ?God?, warfare, power surges and unauthorized personnel work on system. Delivery terms shall be FOB destination to U.S. Naval Hospital, Guam, Bldg. 1, Farenholt RD, Agana Heights, Guam 96910. Equipments to be on island within three (3) weeks of contract award, and installation shall be completed within 45 days of equipment arrival on island. Proposed Contract requirements: Provide 24 hours a day 7-days a week monitoring service availability, 24 hour monitoring of all systems, All materials and labor required to support the Hospital should be included as well as preventive maintenance checks and loaner equipment in the annual contract. There should be no budgetary surprises associated with the normal servicing of the system; all products bought and installed should meet/exceed stringent UL requirements, and are ADA compliant, this also applies to the loaner equipment; the contractor will ensure that all of the security surveillance equipment is installed correctly and in a timely manner as specified on the contract terms and conditions; down time for equipment should be no more than 12 hours (loaner must be in place and working properly as well within this time frame) after the trouble call is placed, this is to include defective equipment as well, equipment damaged by nature shall be replaced within 16-18 hours with a loaner or new part once the trouble call is placed; Response time: The Naval Hospital will be assigned as priority response facility; Authorized Personnel: The Naval Hospital, Guam Security Officer,Chief-Master-At-Arms, or the COD (Chief of the Day?s Desk) are the authorizing personnel for placing trouble calls; Preventive Maintenance: Shall be done every three months or semi-annual. As the contractor, you should be able to implement and maintain cost and schedule monitoring procedures adequately to provide performance reporting timely, identification and resolution of all problems that effect proper completion of this effort. Proposals shall be submitted on an all or none basis. Partial proposals shall be rejected.
