Description
STATEMENT OF WORK VA Medical Center, Minneapolis, MN has a requirement to furnish all labor, equipment and material, which includes selective demolition, for the installation of Medical Vacuum Pumps with mechanical, electrical and other miscellaneous appurtenances.=20 Replacement of Medical and Surgery Vacuum Pump Systems with pumps and motors, electric power, alarm signals, fire stopping and other appurtenance, as defined in the bid schedule, at the Minneapolis VA Medical Center. All work shall be accomplished in accordance with Veterans Affairs Medical Center Project No.618-00-0106, Project Drawings No. 1070, 1070A, 1070B and Specification No. 02-3.=20 BRAND NAME ONLY: The Department of Veterans Affairs Medical Center =96 Minneapolis, MN has determined that compatibility with remaining vacuum system is paramount to maintain continuity under medical/surgical conditions and has designated "Travaini Medical Package Vacuum Systems" as a brand name only product. THIS PROCUREMENT IS 100% SET ASIDE FOR SMALL BUSINESS COMPETITION. BASE BID: Item #1:=20 Removal and disposal of six (6) ea.10hp Medical Vacuum Pumps, VP-1, including connection and reservoir tank, in mechanical room BD-101. Furnish and install two (2) ea. 40hp units, to include mechanical and electrical connections, including one (1) 400g vertical air receiver reservoir tank.=20 Mechanical equipment connections shall be connected to the existing Medical vacuum intake run and roof exhaust pipe lines.=20 Price: _____________ Item #2:=20 Removal and disposal of one (1) ea. duplex Surgery Vacuum Pump, VP-4, including reservoir and control panel, in mechanical room 2V-153. Furnish and install one (1) ea. duplex Surgery Vacuum Pump, including 80-gallon horizontal air receiver reservoir tank, control panel, mechanical and electrical connections. Mechanical equipment connections shall be connected to the existing Surgery vacuum intake run and roof exhaust pipe lines.=20 Price:_____________ Item #3 =96 Add Alternate:=20 Removal and disposal of three (3) ea. 15hp Medical Vacuum Motors, VP-2, in mechanical room BJ-103. Furnish and install three (3) 20hp Medical Vacuum Motors. Motors only shall be connected to the existing three (3) medical vacuum pump units, VP-2, in mechanical room BJ-103. Price: ____________ Contractor must be on-site for start-up and determination of fully operating system. The contractor guarantees the equipment against defective material, workmanship and performance for a period of not less than 1 year or manufacturer's commercial warranty, which ever is greater, and said guarantee is to begin from the date of acceptance by the Government. The contractor agrees to furnish and install, without cost to the Government, at the point of installation, replacement of parts and material which are found to be defective during the guarantee period. All Specifications (No. 02-3) apply to this RFQ 618-160-02.=20 BASIS FOR EVALUATION OF PROPOSALS: Evaluation of proposals may include subjective and objective evaluations on offeror's past performance, technical capability of the equipment, supplies and services offered, and their value to the Government. Award will be made to the offeror that has been determined to provide the =93best value=94 to the Government. Contractor is requested to submit descriptive literature for the equipment offered. The descriptive literature may be used to assist in the evaluation of products offered. All work is to be completed within 90 calendar days following Notice to Proceed. Liquidated Damages will be accessed at $300.00 per calendar day for every day exceeding the completion date.=20 Proposals shall include Descriptive Literature, Representations and Certifications and address technical capabilities, past performance, price and price related factors. PRE-BID CONFERENCE AND SITE WALK IS SCHEDULED FOR: TUESDAY, SEPTEMBER 17, 2002 AT 10:30 A.M. MEET IN ENGINEERING SERVICE, ROOM BR-140, VAMC, Bldg. 70 (main hospital complex).
