Description
Under Statutory Authority 41 U.S.C 253 (1) the Government anticipates entering into sole source procurement negotiations for Brand Name Only Medical Equipment under this combined synopsis and procurement as Request for Quotation RFQ 618-164-02 entitled Brand Name Only CARTO XP ELECTROANATOMICAL MAPPING/NAVIGATION SYSTEM by Biosense Webster, Diamond Bar, CA and EnSite 3000 SYSTEM by Endocardial Solutions, St. Paul, MN; both are arrythmia monitoring equipment for Cardiology Department, VA Medical Center, Minneapolis, MN. This is a combined synopsis/solicitation issued for commercial items and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for commercial Items, as supplemented with additional information included in this notice. This procurement is being conducted under FAR Subpart 13.5 test program; simplified acquisition procedures apply and VA will conduct the procurement according to a combination of Parts 12 and 13. This announcement constitutes the only solicitation; a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-09. NAICS code 421450, size standard of 100 employees applies. Department of Veterans Affairs is seeking (1) Brand Name only CARTO XP System as follows: Model M-4700-01 with location pad, with foot pedal; grounding cable (8 foot); power supply cord; optical cable; patient cable adapter; patient cable; Windows NT Workstation Computer with flat screen repeater monitor, 18=94 high resolution, distribution amplifier, Anthro Cart (2 ea), 2 Operator manuals, HP 890C inkjet printer; training for two physicians, and Biomedical Technician Training for two (2); 2 Operator Manuals and 2 Service Data Manuals manuals. Second arrythmia monitoring system is EnSite 3000 System with second monitor, ESI custom carts and printer; two Operator Manuals and two Service Data Manuals and a minimum of 5 EnSite Catheters. Cardiology=92s estimate is one procedure per month; provide warranty provisions. Vendors will provide on-site assistance and training during each case for at least the first 6 cases with each piece of equipment. This purchase hereby incorporates provisions and clauses in effect through FAR FAC 2001-05 and VAAR 97-9. Contractor's proposals are hereby being requested in response to requirements listed and shall be submitted on contractor's letterhead and must address all requirements listed. Contractor's response should contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor.=20 Descriptive literature for each item offered will be included in proposal/bid package. Include Y2K Compliance warranty. Delivery shall be FOB Destination (unless vendor states otherwise) to VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417.=20 Award will be firm-fixed price; payment may be made by Government Purchase Card (VISA). Evaluations may be conducted to determine that the product(s) offered meet Government's requirements, technical capability and past performance history, delivery time frame and warranty to determine "best value" to the Government. Evaluation and/or award may be made without discussions. All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead.
