Description
The Aberdeen Area Indian Health Service (IHS) is soliciting offers from interested firms to provide Dialysis Service for the Indian Health Service Standing Rock Service Unit. The successful contractor will be capable of providing accessible, high quality dialysis services to IHS beneficiaries served by the Standing Rock Service Unit and other federally recognized Indians with end-stage renal disease. The IHS anticipates the award of a fixed price "Indefinite Delivery/Indefinite Quantity" type contract. The period of performance will be for one (1) base year and four (4) option years. Contract performance will start on the earliest date that a "full and open competition" can be completed and an award is made. The government will reserve the right to exercise the option years. RFP No. 02-16 will be released on October 16, 2002 and the deadline for submission of offers is tentatively scheduled for November 18, 2002. PUBLIC NOTICE OF PRESOLICITATION CONFERENCE: The public is hereby notified that the Aberdeen Area Indian Health Service and Standing Rock Service Unit invites interested end users, potential offerors, and government support personnel to participate in a Pre-solicitation Conference to exchange information relating to the "Statement of Work" for this project. The Pre-solicitation Conference will be held on September 26th, 2002 at the Prairie Knights Casino and Resort, 7932 Highway 24, Fort Yates, North Dakota 58538, call (800) 425-8277 for hotel reservations. The conference will start at 8:30AM and end at 5:00PM CST. The Indian Health Serivce will not be responsible for nor reimburse participants for attendance or travel expenses to participate in the this conference. The purpose of this conference is as follows: 1. The purpose of exchanging information to improve the understanding of Government requirements and vendor capabilities thereby allowing potential offerors to judge whether or how they can satisfy the Government's requirements, and enhancing the Government's ability to obtain quality medical services at reasonable prices, and increase efficiency in proposal preparation, proposal evaluation, negotiation and contract award. (See FAR Part 15.201 for legal authority) 2. The key subjects of discussion will include: Term and conditions, performance requirements, Statement of Work, contract type, IHS credentialing requirements, employee qualification, evaluation criteria, oral presentation, past performance, invoicing/payment policies and procedures and provider concerns. We will allow a "full and open" question and answer session. Following is the Draft Statement of Work (SOW) for Dialysis for the Standing Rock Service Unit: Statement of Work Dialysis Services for the Standing Rock Service Unit in the Aberdeen Area Indian Health Service Purpose of the Contract The mission of the Standing Rock Service Unit is to ensure that the highest quality of direct and preventative care is provided to all American Indians. We place our highest value in elevating the health status of all eligible patients on the Standing Rock Sioux Reservation. To meet the mission, the Standing Rock Service Unit requires contractor support to provide accessible, high quality dialysis services to patients of the Standing Rock Sioux Tribe and other Federally recognized Indians with end-stage renal disease. The ESRD provider hereinafter referred to as the "Contractor" shall furnish the necessary space, supplies, equipment, and personnel to provide high quality dialysis services to meet the needs of the Standing Rock Service Unit hereinafter referred to as the "Government". The Government requires dialysis services continue without interruption during the term of this contract. The Contract resulting from this solicitation shall: 1. Be a non-personal health care services contract under which the Contractor is an independent contractor; 2. Allow the Government to evaluate the quality of professional and administrative services provided; 3. Require the contractor to indemnify the Government for any liability producing act or omission by the contractor, its employees and agents occurring during the contract performance; 4. Require the Contractor to maintain medical liability insurance, in the coverage amount acceptable to the Contracting Officer, which is not less than the amount normally prevailing within the local community for the medical specialty concerned, and 5. Require the Contractor to assume sole and exclusive responsibility for employee federal taxes, social security, unemployment insurance, and workman's compensation. Government Furnished Property, Facilities, and Services The Contractor will be authorized to use all areas of the medical facility in which they have business and patient care related functions. The same restrictions apply to the Contractor as any other Indian Health Service personnel to use these areas for official business activities only. Each Contractor staff member shall be provided with an orientation to the Standing Rock Service Unit and the Indian community being served. The initial orientation and continuing orientation shall be provided by the Service Unit Human Resouce Director or his/her designated representative. The Standing Rock Service Unit shall provide all required blank government forms applicable to the Indian Health Service. An appropriate orientation to the necessity of such forms will be provided by the Property and Supply Supervisor or his/her designated representative. The Standing Rock Service Unit shall provide the authority for Contract staff to purchase meals in the service unit dining room when available. The same IHS restrictions to use this service apply. The Standing Rock Service Unit shall provide an ongoing method of monitoring and evaluation of the quality and appropriateness of patient care and of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable. The Standing Rock Service Unit shall furnish janitorial services, snow removal and grounds keeping to assure that the grounds and walkways to the Dialysis Unit provide safe access for patients, visitors, and employees working at the Dialysis Unit. The Standing Rock Service Unit shall furnish one (1) meal per dialysis treatment for any patient dialyzing in the Dialysis Unit who is eligible for Indian Health Services. Disposal of trash generated by the Dialysis Unit, however, the trash will be brought to an appropriate location designated by the service unit. All transportation of dialysis patients. The Standing Rock Service Unit shall be responsible for replacing items from the crash cart along with the maintenance of the crash cart. The Standing Rock Service Unit shall provide medical care to all eligible hemodialysis patients receiving treatment in the Dialysis Unit and continues its responsibility to these patients as mandated by Federal Regulation as well emergency lab and radiology serivces. Contractor Furnished Equipment, Facilities, Supplies & Services The Contractor shall provide a structure in which dialysis services are performed and the maintenance of this structure. Scheduling of and costs of all necessary maintenance and repairs shall be the responsibility of the Contractor. The Contractor shall provide all necessary personnel, supplies, drugs, equipment, laboratory, facilities, water treatment system, and other items necessary for providing dialysis services. Maintenance and repairs fo all equipment shall be the responsibility of the Contractor. The Contractor shall provide qualified personnel including a Medical Director, (Nephrologist) for the Dialysis Unit, ACLS certified professional nursing staff, and ancillary services including bimedical technician, dietitian, social services, and dialysis technical services. It is encouraged that some of the employees shall be Lakota speaking who shall provide translation for Lakota speaking patients who are being treated in the unit. Statement of Work 1. The Contractor shall provide End Stage Renal Disease (ERSD) Hemodialysis Services. This may include continuous cycling peritoneal dialysis (CCPD), home dialysis training and other necessary services considered a routine part of ESRD health care. These services shall be provided in accordance with the Centers for Medicare and Medicaid (CMS) standards as well as the Joint Commission on Accreditation of Healthcare Organizations standards. Contract performance shall be according to the requirements contained in this Statement of Work. 2. The standards of medical practice and duties of the Contractor through its dialysis services shall be determined pursuant to the bylaws of the appropriated IHS hospital, the regulations of the hospital, applicable provisions of law, other rules and regulations of any and all government authorities relating to licensure and regulation of physicians and hospitals, and the standards and recommendations of the Joint Commission on Accerditation of Healthcare Organizations. The Contractor shall be required to participate in the hospital-based performance improvement program which is consistent with current JCAHO and CMS requirements for ongoing monitoring and evaluation of the quality and appropriateness of care. 3. The Dialysis Unit shall be a Contractor owned free-standing unit, which shall be located within 5 minutes of the Standing Rock IHS Hospital, maintain Medicare, Medicaid certification as well as JCAHO accreditation. Failure to maintain certification with CMS and accreditation with JCAHO will be cause for immediate termination of this contract. 4. The referring medical staff of IHS will be the primary care providers of all other medical care other than ERSD conditions. 5. All hospitalizations and any non-routine ERSD laboratory tests not included in the Chronic Dialysis Treatment Fee are to be referred to the IHS hospital as capable of providing the necessary care/lab tests. The IHS hospital's Clinical Director will be the local contact and coordinator for referrals, patient emergencies, treatment, and consultation. 6. The Contractor's Medical Director shall be a Nephrologist. The Contractor shll require the patient's attending Nephrologist to be present in the dialysis unit at least twice a month to monitor the progress of each patient and coordinate patient's care with the Service Unit's Clinical Director. The Contractor shall ensure continuity of care through collaboration between the attending Nephrologist and the referring medical staff. 7. The Contractor shall have a plan to improve access to renal transplantation and home dialysis. 8. The Contractor shall include the IHS Clinical Director and/or his/her designee to attend patient care conferences. A quarterly report shall be submitted to the Clinical Director and the IHS CEO on the health status of the dialysis patients and the overall function of the Dialysis Unit. 9. The Contractor shall provide a long term plan for providing treatment within a specified setting based on the needs of the patient, demonstrating the most suitable treatment modality (dialysis or transplantation) based on medical, nurtritional, and social evaluation of the patient. A copy of this plan shall be provided to the Standing Rock Service Unit for inclusion in the patient's medical record. 10. The Contractor shall provide patient amenities (i.e. television, reading materials, etc.) for hemodialysis patients. 11. The Contractor shall provide and maintain for the duration of this contract the services of a qualified Medical Social Worker, who is experienced in working with dialysis patients and who will cooperate fully with IHS Social Service staff in coordinating services and program benefits. Required Social Services assistance under this contract includes identifying alternate resource programs which may offer services and payment assistance which ESRD patients may be eligible for and assisting these patients to complete the applications to these programs. The Social Worker shall participate in periodic care conferences as requested by the Nephrologist or Service Unit's Clinical Director. 12. The Contractor shall have a Performance Improvement Plan which is integrated with the Standing Rock Service Unit Performance Improvement Plan. One focused study will be patient satisfaction. The Contractor shall coordinate these efforts with the Standing Rock Service Unit Performance Improvement Coordinator. A copy of the patient grievance procedure shall be provided to the Standing Rock Service Unit. 13. The Contractor shall perform biological and chemical tests in accordance with the standards set forth by the Association for the Advancement of Medical Instrumentation and the American National Standards Institute. 14. The Contractor shall allow full access to the dialysis unit for Environment of Care staff and patient's records shall be available to the Service Unit's Clinical Director, Medical Staff and Performance Improvement Coordinator. 15. The Contractor shall follow all HIPAA and Privacy Act Regulations. Any violations will result in civil and criminal penalties as set forth in these laws and regulations. 16. The Contractor shall be required to maintain records that document competence/performance level of Contractor employees working in the dialysis unit in accordance with JCAHO, CMS, and other regulatory body requirements. The Contractor shall provide a copy of the competence assessment and checklist and annual performance evaluation to the Standing Rock Service Unit project officer for each employee working under this contract. Other Contractor Responsibilities 1. The Contractor shall participate in a continual process of self-evaluation and strive for excellence in dialysis services. 2. The Contractor shall respect and maintain the basic right's of patients, to include but not be limited to personal dignity. 3. The Contractor shall deliver services in a culturally sensitive manner. 4. The Contractor shall develop a holistic plan of care to meet each individual's needs including physical, psychosocial, spiritual, and cultural values and sensitivities. Contractor Fees and Reimbursement 1. The Contractor shall be allowed to bill and be reimbursed for ERSD services according to the Medicare reimbursement methodology. 2. Reimbursement to the Contractor for a new patient awaiting Medicare eligibility will be according to Medicare reimbursement methodology. 3. Reimbursement to the Contractor for Acute Inpatient Dialyzed at the Contractor's Dialysis Unit will be according to the Medicare reimbursement methodology.
