- NA This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are be ing requested and a written solicitation will not be issued. The solicitation number is: DAAH01-02-T-R012, the solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. The contract line item number(s) and items, quantities and units of measure are: CLIN 0001 - Repair service of one (1) job to replace of Beam Centerline on the Linatron, s.n. 014, 1000A x-ray system. Descript ion of requirement for the repair service to be provided is as follows: Repair is required to the Linatron 1000A x-ray system by replacing the Beam Centerline to restore the systems capabilities to the original performance specification. The requested service date shall be 30 Nov 02. Inspection/Acceptance/FOB will be destination. The place of performance is: Building 7290, ATTN: Charlie J. Beyer, 256-955-7056, CSTE-DTC-RT-M-CL, Redstone Arsenal, Alabama 35898. All responsible sources may submit an offer, which shall be considered by the Agency. Justification for other than full and open competition is FAR 6.302-1, Only One Source or Only a Limited Number of Responsible Sources. This proposed contract action is for repair for which the Government does not have specifications and drawings to permit full and open competition. Sole source for this service is: Varian Medical Systems. Any award resulting from this RFQ will be issued on Standard Form 1449 and will contain all clauses required by law , the Federal Acquisition Regulation (FAR), and the Defense Federal Acquisition Regulation Supplement (DFARS) as appropriate for the dollar value of the award. Provision 52.212-1, Instructions to Offerors - Commercial; Clause 52.212-3, Offer Representatio ns and Certifications - Commercial Items; Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, are applicable to this ac quisition. Offer is due not later than 12:00 noon, Central Time, 13 Sep 02, in the contracting office, Building 5400, Room B145, Redstone Arsenal, Alabama 35898-5250. Electronic quotes/offers are acceptable. Quotes/offers received after the specified du e date will not be considered for award.
