- Description
- The Amarillo VA Health Care System seeks non-personal services as follows: Provide all labor, supplies, materials, equipment and parts necessary for preventative and corrective maintenance on the ultrasound equipment located at 6010 Amarillo Blvd. West, Amarillo, Texas. All work is to be performed during normal working hours: Monday-Friday, 8:00am - 4:30pm, excluding Federal holidays. During the term of this agreement two preventative maintenance procedures will be scheduled and completed at approximately six-month intervals. These shall consist of electrical and mechanical inspections designed to identify systems or subsystems that require further service or maintenance. All recommended calibration/adjustment procedures will be completed during these visits. The VA will receive a service report each time the unit is serviced. This report will identify all parts used and work completed and time required for this service. Equipment as follows: 001)ATL Ultrasound Unit, Model UM9DC, s/n V104479; Video Recorder Model 113, s/n EITH000073; Matrix Camera, Model 1010, s/n 16131751091; Transducer, Model CVLA/3.5, s/n EITH00073; Transducer, Model IVT/3.5, s/n 91K0006; Transducer, Model ICT/7.5, s/n 91K0010; Transducer, Model ICT/3.5, s/n 92C5018; Color Printer, Model UP5000, s/n 0065839-H; 002) Ultrasound Mark 9, Model UM-9-OPT 01, s/n K-101383 to include video recorder matrix camera and color printer; Transducer, Model LA 5.0 MHZ, s/n 001568; Transducer-Annular Array, 3.5 MHXZ, s/n 09411; Transducer-Convex Array, 3.5 MHZ, s/n 91F40356; Transducer-Alt/ADR Vaginal, 5.0 MHZ Period of contract: October 1, 2002 through September 30, 2003 with two one-year option periods.
