- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propsals are being reqested and a written solicitation will not be issued. F09604-02-R-00640 is issued as a request for proposal(RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. F09604-02-R-00640 is a sole source procurement to Olmpus America, Inc. The associated NAICS code is 334513. The Small Business Competitiveness Demonstration Program is not applicable to this procurement. The items to be procured are as follows: Line Item 0001, 6MM IPLEX SYSTEM-NTSC, item number N6954242,4 EACH Line Item 0002, 60/D STEREO TIP-MAJ-104D, item number N6956400, 4 EACH Line Item 0003, 60S/ STEREO TIP-MAJ-104S, item number N6956500, 4 Each Line Item 0004, FAR FOCUS DIRECT VIEW TIP, item number N6955200, 4 EACH Line Item 0005, FAR FOCUS SIDE VIEW TIP, item number N695500, 4 EACH Line Item 0006, 3D EYETREK FMD-3 NTSC, item number N1015830, 4 EACH Line Item 0007, RIGID SLEEVE/IV76 IPLEX, item number N1015700, 4 EACH. The items to be acquired are to be new items. Surplus and refurbished items are not exceptable for this requirement. Delivery will be 45 days After reciept of Contract. Items will be ship to FX2399. Acceptence and Inspection at origin. FOB point will be orgin. Provision 52.212-1, Instructions to Offerors--Comercial, applies to this acquisition. Addenda are not included. Clause 52.212-2 is not applicable. Offeror(s) are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items, with its offer.
