Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information in this notice. This procurement is being conducted under the Simplified Acquisition Procedures in accordance with FAR Part 13 and Part 15. This announcement constitutes a Request for Proposal (RFP). A written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through FAC 01-08. RFP 659-24-03 shall be referenced on all proposals. This acquisition is 100% small business set-aside. The NACIS Code is 492110 with a small business size standard of 1,500 emplyees. The VA intends to solicit proposals and award a firm-fixed price contract for courier service to pick up lab specimens at the Charlotte CBOC, 8401 Medical Plaza Dr., Charlotte, NC and deliver to the W. G. "Bill" Hefner VA Medical Center, 1601 Brenner Ave., Bldg 2, R. Pickup time in Charlotte is between 4P and 4:15P with delivery to Salisbury no later than 6P the same day. This schedule is not flexible. Any delays for reasons beyond the control of either party, such as natural disaster, dangerous driving conditions,highway blockage due to accidents or road work or any other reasonable delay beyond the control of either party, the terms of delivery will be negotiated between the parties to assure mutual satisfaction of both parties. When such a delay is encountered, the Contractor's driver will call the pick-up site or delivery site, whichever is applicable to advise of approximate arrival time. There will be no pickups on Government holidays. Government holidays are New Year's Day; President's Day, Martin Luther King's Birthday; Memorial Day, July 4th, Labor Day, Columbus Day, Veterans' Day; Thanksgiving Day and Christmas Day. This contract is for a base year plus four (4) one-year renewal options. There is one (1) contract line item number for each year, including options. Firms submitting proposals should submit as follows: Base Year (10/1/02-9/30/03) - CLIN 1 - daily charge to deliver as specified in Statement of Work $_____; OY 1 (10/1/03-9/30/04)- CLIN 1 - daily charge to deliver as specified in Statement of Work; OY 2 (10/1/04-9/30/05) CLIN 1 - daily charge to deliver as specified in Statement of Work; OY 3 (10/1/05-9/30/06) CLIN 1 - daily charge to deliver as specified in Statement of Work and OY 4 (10/1/06-9/30/07) CLIN 1 - daily charge to deliver as specified in Statement of Work. The provisions at 52.212-1 Instructions to Offerors - Commercial, applies to this acquisition as well as any addenda to this provision. FAR 52.212-2, Evaluation -Commercial Items, as included. Evaluation criteria is: past history is providing courier service including contract locations and a point of contact for each; technical requirements of the contract, specifically ability to meet the daily time constraints of pick-up and delivery and cost. All evaluation criteria are considered equal. The Government will make award based on best value to the Government, price and other factors considered. FAR 52.212-3, Offeror Representations and Certifications - Commercial Items is to be completed and returned with the offer. Contract Terms and Conditions - Commercial Items (FAR 52.212-4)applies as well as any addenda to this clause. FAR 52.212-5 Contract Terms and COnditions Required to Implement Statutes or executive Orders - Commercial Items, applies to this acquisition. This includes FAR 52.222-26, 52.232-33; 52.222-41 and 52.222-43. Proposals are due by close of business (3:30pm) September 27, 2002. All proposals shall be submitted to the Contracting Officer (40P) Hefner VAMC, 1601 Brenner Ave., Salisbury, NC 28144 ATTN: Jolette Cole. Faxed proposals are not allowed. Firms submitting proposals must also provide copies of general liability and insurance certificates with their proposal.
