- Description
- This is a combined synopsis/ solicitation. The VA Network Business Center intends to negotiate a commercial sole source contract with Given Imaging LTD, 5555 Oakbrook Parkway # 335, Norcross, GA 30093, under the authority of FAR 6.302-1. Only one responsible source and no other supplies or services will satisfy agency requirements. The Request for Quote 600-207-02BV entitled "Capsule Endoscopy System" is hereby issued for Commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2001-08. This procurement is being conducted under the Simplified Acquisition Procedures in accordance with Federal Acquisition Regulation (FAR) Part 13. The North American Industry Classification Code is 421450 and the small business size standard is considered 100 employees. This BOS announcement constitutes the only solicitation issued. Given Imaging, LTD shall supply the following items: (1) (Stock Number 40040) Capsule Endosopy System to include workstation kit, computer workstation with 17=94 color display, keyboard and mouse, color inkjet printer, rapid applicator, software license 1 per workstation, system accessories, cables, adapters, connection box, documentation, (2) (Stock Number 4000) Data Recorder Kit to include data recorder, recorder belt, sensor array, battery charger, rechargeable battery packs, carrying case. (3) (Stock Number 20451) 10 M2A PAK KIT to include 10 single use M2A capsules, 10 applicators kits each containing 10 sensor array sleeves, two adhesive pads, one sensory template, ten disposal razors, one marking pen. All proposals received by 4:00 PM Pacific Daylight Time, September 11, 2002, will be considered by the Government. A determination by the Government not to compete this proposed contract based on responses to this notice is solely with the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Inquires will only be accepted by facsimile at (562) 826-5828. Delivery terms shall be FOB Destination to the VA Long Beach Healthcare System, Warehouse Building 149, 5901 East Seventh Street, Long Beach, California 90822. Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (FEB 2000). (In addition, incorporated herein by reference are the following VA Acquisition Regulations (VAAR) clauses and provisions: 852.211-70 Service Data Manuals (Nov 1984), 852.211-71 Guarantee (Nov 1984); 852.270-1 Commercial Advertising (April 1984), and FAR Provision 52.215-5, Facsimile Proposals, is hereby included.
