- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Solicitation F64133-02-R-0025 is being issued as a Request For Proposal (RFP). (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 01-08. (IV) This requirement is being synopsized as a 100% small business set-aside under standard industrial classification code 7389, NAICS Code 54199, small business size standard of $6.0 Million. (V) Please propose the service as listed: CLIN 0001, 1 each. The contractor shall provide Coding personnel to furnish ambulatory coding training, conduct ambulatory record coding assessments, provide feedback to professional staff, coordinate and implement data collection tool improvements, answer coding questions, and code complex ambulatory medical records for the base period of 30 Sep 02 through 29 Sep 03 and for 4 option years. The contractor shall provide services in Government supplied facilities at the 36th Medical Group, Andersen AFB, Guam, also referred to as the Medical Treatment Facility (MTF) herein. Services shall cover the range of ambulatory coding services typically provided in a multi-speciality civilian practice. Performance shall be according to the requirements contained in the Performance Work Statement (PWS). (IX) Provision 52-212-2, Evaluation - Commercial Items applies. Proposal will be evaluated on technical capability, price, and past performance. (X) Offerors are required to submit a completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items with their offer. (XI) Clause 52.212-4, Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to the award document. (XIII) Wage Determination number 94-2147 Rev (19) will be used. (XIV) N/A. (XV) N/A. (XVI) A site visit will not be held.
