- Description: This is a combined synopsis/solicitation. The VA Network Business Center, Long Beach, California intends to award a firm fixed price unrestricted contract. The Request for Quote 600-169-02BV entitled "Surgical Table with Accessories" is hereby issued for Commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2001-08. This procurement is being conducted under the Simplified Acquisition Procedures in accordance with Federal Acquisition Regulation (FAR) Part 13. The North American Industry Classification Code is 421450 and the small business size standard is considered 100 employees. This BOS announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. The VA Long Beach Healthcare System is seeking a contractor to provide a brand name or equal for the follow equipment. NOTE: ALL THE FOLLOWING EQUIPMENT IS RELATED AND INTERLOCKS. (1) (Stock # ST010201) Table AMSCO 3085 Major Surgical Table: The surgical table must be available in four configurations both battery and electric, the versions are available with or without x-ray tops. The table must have the ability to be used with a c-arm. The surgical table shall have four-section top (approximately ?75L x 20? W) that is radio translucent and shall have a desirable manual kidney elevator. Table shall have the hydraulic actuation that enables ultra-precise tabletop articulation. Also, must have the auxiliary control and manual foot pump back up normal hand and foot controls to assure table motions during power outages. Offeror shall provide an easily programmable hand control with normal reverse patient positioning. Offeror must provide a tabletop height that is adjustable from 27? to 44?. Offeror shall provide the stainless steal covered ?T shaped? table base and column enable that is optimum surgical team and equipment interface. The base shall include four swivel casters and self-leveling floor locks. (2) (Stock # BF41130) Standard Accessory Package: The accessory package shall include: pair of Anesthesia Armboards with 3? Pads, a Perennial Cutout Filter Piece a Cable Wrap and Conductive Restraint Straps. The equipment shall provide auto-lift multi stirrups safe system for positioning patients in a variety of lithotomy positions. Must have power-assisted positioning as well as auto-abduction that assures proper positioning if the patient in three fast easy steps. (Also for 1050 M and 2000L tables). (3) (Stock # BF 0830) CLARK SOCKETS: The sockets must be ideal for mounting accessories anywhere along the tableside rail. These sockets shall provide angular control and positive locking for an accessory in a plane parallel to side rail. The sockets shall have securing accessories with round posts. The offeror shall provide easily removable sockets that are compatible with OrthoVision, (Also for 1050 M and 2000L tables). (4) (Stock # BF4410) AUTO-LIFT MULTI-PURPPOSE STRIRRUPS; These stirrups shall have a safety system for positioning patients in a variety of lithotomy position. Offeror must provide power-assisted positing as well as auto-abduction which assures proper positing of the patient in three fast, easy steps. Proposals shall be submitted on an "all or none basis." Partial proposals shall be rejected. If submitting on a brand name equal basis, sufficient technical data must be included for the government's evaluation. Delivery terms shall be FOB Destination to the VA Long Beach Healthcare System, Warehouse Building 149, 5901 East Seventh Street, Long Beach, California 90822. Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (FEB 2000).
