PART: U.S. GOVERNMENT PROCUREMENTS SUBPART: SUPPLIES, EQUIPMENT & MATERIAL CLASSCOD: 65—MEDICAL, DENTAL & VETERINARY EQUIPMENT & SUPPLIES OFFADD: Department of Veterans Affairs, Contracting Officer 90C, VA Medical Center, Building 41, 2200 Fort Roots Drive, North Little Rock, AR 72114-1706 SUBJECT: NOTICE OF INTENT TO MAKE A SOLE SOURCE AWARD— SERVICE AGREEMENT NUCLEAR CAMERAS SOL: 598-101-02 POC: Donna Landrum DESC: Notice of Intent to make a sole source award to Philips Medical Systems for full service on Picker Nuclear Cameras, Picker Prism 2000 XP Nuclear Camera, S/N# 649 and Picker Prism 3000 Nuclear Camera, S/N# 185. Philips Medical Systems shall provide all necessary personnel, equipment, materials, supervision and labor required to maintain these the government owned equipment listed in this announcement in operating order in accordance with all manufacturer specifications for the period of October 1,2002 through September 30, 2003. Equipment is located at John L. McClellan Veterans Memorial Hospital, 4300 West 7th Street, Little Rock, AR 72205. These requirements and specifications are to include (on site when possible) the complete servicing but not limited to the following to assure equipment is maintained and performs in accordance with the manufacturer’s specifications. Labor to include troubleshooting, repairs, parts replacement for unscheduled, emergency and preventive maintenance. Replace all worn, damaged, or spent parts with original manufacturer’s replacement parts (new or warranted). Scheduled preventive maintenance is to be performed in October, January, April and July of each contract period and shall include but not be limited to: technical inspection, cleaning, lubricating, adjusting, calibrating or replacing all worn, damaged or expendable parts in accordance with manufacturer’s recommended preventive maintenance guideline, Federal, State, and Local regulations. Philips Medical Systems must provide documented proof of qualifications (ie: manufacturer’s training or prior experience of equipment covered by the contract). Philips Medical Systems must be in possession of all technical service manuals including schematics, calibration procedures etc. Philips Medical Systems must provide at no additional charge all software updates, upgrades and patches as offered by OEM. Philips Medical System must be in possession of all specialized test equipment needed to perform all maintenance in accordance with all appropriate service manuals.
