- NAICS Code: 514210 THIS REQUIREMENT IS RESTRICTED TO SMALL BUSNESSES ONLY. STATEMENT OF WORK I. Summary This service will be performed in the State of Wyoming. The goal of this service to a) develop an electronic WordPerfect and Microsoft Word emergency response plan template for Wyoming public water systems serving greater than 3,300 population and, as remaining funds allow, b) to adapt the template for interested public water systems. Public water system emergencies are defined as loss of water supply, and gross microbiological, chemical or radiological contamination of a water supply. Gross contamination is defined as contamination that exceeds the 10-day health advisory for a 10 kg child. The specific tasks that have been identified are listed below in Part II. Quotations from interested contractors should be submitted with cost estimates (hourly rate) and time frames (number of hours estimated) for performing each task. To perform this requirement successfully, the contractor will need to become thoroughly familiar with the American Water Works Association (AWWA) guidelines for the development of public water system emergency response plans and the Wyoming Public Water Supply Direct Implementation Emergency Response Plan. The contractor will be chosen based on the following criteria: 1. Demonstration of knowledge and abilities for A & B listed below. 2. Past Performance: Please list at least three (3) references of similar work, including project contact and phone number. 3. Price. Please provide the number of hours anticipated to perform each Task listed in II. A and B and the hourly rate for each Task. A.Knowledge of the American Water Works Association guidelines for the development of public water system Emergency Response Plans. For example, training (information including dates, location, audience mix and student evaluations) on the subject provided by the prospective contractor. B.Knowledge of public water system operation and maintenance and small government public works administration. II.Specific list of tasks and services the contractor will perform: A. DEVELOP ELECTRONIC TEMPLATE. 1.Use AWWA guidelines as reference. 2.Develop in WordPerfect and Microsoft Word. 3.Submit draft to EPA Work Assignment Manager within one month of project start date. 4.Complete final product within three months of project start date. B.WORK WITH SELECT PUBLIC WATER SYSTEMS (EPA will market service and provide contact names to contractor) to customize the Emergency Response Plan template for these systems. This Request for Quotations (RFQ) is issued by the Simplified Acquisition Office of US EPA Region 8, Denver, CO. Refer to FAR Part 13 for Simplified Acquisition Procedures.
