- Contractor to furnish 24 hours, 7 days per week Ambulette Service to the following listed two (2) Veterans Affairs New Jersey Health Care Systems and their six (6) Community Based Outpatient Clinics for the period from Date of Award through September 30, 2003 with the option to extend the term of the contract for two (2) one year periods, in strict accordance with the specifications, requirements, terms, conditions and schedule as described within solicitation RFP 10N3-229-02. VANJHCS: 385 Tremont Avenue, East Orange, NJ 07018, 151 Knollcroft Road, Lyons, NJ 07939, 317 George Street, New Brunswick, NJ 08901, 115 Christoper Columbus Drive, Jersey City, NJ 07302, 654 E. Jersey St., Elizabeth, NJ 07201, 171 Jersey STreet, Trenton, NJ 08611, 382 Prospect Avenue, Hackensack, NJ 07601 and 970 Route 70, Brick, NJ 08724. To qualify for award of this solicitation/contract, ambulette units are to be housed and dispatched from a location within thirty (30) miles radius of the VANJHCS's and CBOC's installations. NAICS 485991, Size Standard $6.0. Issue date of solicitation RFP 10N3-229-02 is August 29, 2002. Due date for Proposal is September 30, 2002.
