- Description
- The Department of Veterans Affairs Medical center, San Juan, Puerto Rico 00921-3201, is soliciting for INTERVENTIONAL CARDIOLOGY services. The North American Industry Classification System (NAICS) code 621111 and size standard $8.5 are applicable. This procurement is 100% set aside for small businesses, to provide all services necessary to perform Interventional Cardiology procedures to eligible VA beneficiaries. The contract resulting from this procurement will have duration of one base period, from date of award to the end of current fiscal year (09/30/02), and two additional option years. The solicitation number is IFB672-008-02, and the solicitation is issued as an invitation for bid (IFB). Offerors shall submit all technical questions regarding this solicitation to the Contracting Officer, in writing, no later than 7 days after issuance of this Invitation for Bids (IFB). Questions may be sent via facsimile. Verbal questions will not be addressed. All responses to written questions, which may affect offers, will be incorporated into a written amendment to the solicitation. Requests for the solicitation package must be in writing (letter), or via facsimile to (787) 641-8364, or via e-mail to rene.esteves@med.va.gov. IFB solicitation packages are currently available. IFB solicitation packages shall not be faxed to requestors. Sealed, written offers shall be referred to: VETERANS AFFAIRS MEDICAL CENTER, 10 CALLE CASIA, SAN JUAN, PR 00921-3201 ? Att. Ren? G. Esteves (672/BO-3). The solicitation number shall be listed on the outside cover of the sealed offer for proper identification.
