- Description
- Commerical Item Contract using FAR part 12 & 15 to provide full service maintenance and filling of developer and fixer in film processors for x-ray machines located at the VA Salt Lake City Health Care System, 500 Foothill Blvd. Salt Lake City, UT 84148. Contract to begin 10/1/02 with first year ending at 9/30/03. Contract will have the four option years availiable beyond base year. Proposal documents available on or about August 23, 2002. All request for proposal must be in writing referencing 660-065-02, Attn: Garth M. Glenn, Acceptable forms are fax, letter, and e-mail.
