Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribe in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North American Industry classification system (NAICS) code applicable to this solicitation is 421450, Medical Dental and Hospital Equipment Supplies, Wholesalers, and the small business size standard is 100 employees. This is a full and open competition (unrestricted). The VA Healthcare Network Upstate New York at Syracuse intends to purchase a Kay Elemetrics digital swallowing workstation, Model 7200, brand name or equal, for real-time display, measurement and real-time visual feedback of swallowing signals. In addition to Model 7200 major components include Model #7245 Digital video computer module; model 7120-B swallowing signals lab hardware and software and model 7271 cart/isolation transformer. Salient characteristics of the equipment are: It is an integrated system that allows concurrent acquisition and display of physiological data acquired through swallowing signals lab on same screen as video recording; quick, precise field-accurate correlation of video and physiologic data high fidelity powered speaker and lapel microphone for narration of recordings. Contains signal acquisition module, data acquisition card and windows-based software for real-time display, measurement and real-time visual feedback of swallowing signals. The signals can be displayed simultaneously with the video exam and analyzed with frame-specific accuracy post-acquisition. Permits feedback of pertinent swallow information, e.g., EMG data to show where the person is in relation to the timing of the swallow so that he can withold his swallow or go ahead. Gives information as to tongue strength and feedback from that so that the speech pathologist can monitor changes as exercises are applied. Auscultation feedback of the pharyngeal sounds of the swallow through speakers so that the SLP has further cues as the normalcy of the swallow. Allows hook up to x-ray equipment for direct recording. Allows direct recording of the nasendescopic exam for slow motion recall. It has ports for the addition of other feedback equipment such as manometry for future consideration. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses, in effect through Federal Acquisition Circular 2001-08, and VA Acquisition Regulation (VAAR) clauses as follows: 52.211-6 brand name or equal; 52.212-1 Instructions to offerors commercial items; 52.212-3 Offeror Representations and Certifications commercial items; 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5 contract Terms and Conditions required to Implement Statutes or Executive Orders Commercial itms [with (b)(11), (12), (13), (14), (15), (16), (19)(ii), (21) and (25) checked]; and 852.270-4 Commercial advertising. The provision at 52.212-2, Evaluation of commercial items, applies to this acquisition. The evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the item offered to meet the Government Requirement, (2) past performance and (3) price. The system offered must meet all the salient characteristics specified above to be considered for awrd. Award will be made to the offeror proposing an acceptable system at the lowest overall price.
