- Description: (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Requisition number FQ487721710100 is being issued as a Request for Quotation. (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 01-08. (IV) This requirement is being synopsized as a small business set-aside under the North American Industry Classification System code 235810 and the Standard Industrial Classification code 1781, size standard $12,000,000.00. (V) Line item 0001: Test well 13A, Line item 0002: Test well 14, Line item 0003: Test bowl assembly for both s wells, Line item 0004: Reimbursement for equipment Line item 0005: Sonar jet clean for well 13A, Line item 0006: Video well 13A after Sonar test, Line item 0007: Dislodge rebar and debris and perform a video for well 14 (VI) Contractor must provide all management, tools, supplies, labor, equipment to perform a step-test for production purposes. All work must confirm to the Statement of Work and IAW Standards, Unified Facilities Criteria (UFC) 3-600-02. All new equipment purchased for this requirement shall become government property as permanent installation for either well after test performance. All offerors shall contact SSgt Gomez or SSgt Williams to obtain a copy of the Statement of Work. (VII) Period of performance is estimated at one to two months; place of performance is Well 13A and 14 Davis-Monthan AFB, Arizona 85707, FOB point is destination. (VIII) Provision 52.212-1 Instructions to Offerors- Commercial Items applies. (IX) Provision 52.212-2 Evaluation- Commercial Items applies. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. (X) Offerors are required to request a copy of the 52.212-3 Offerors Representations and Certifications- Commercial Items, from the Contracting Office and submit a completed copy with its offer. (XI) Clause 52.212-4 Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items, applies to the award document. (XVI) A site visit will be held 8 August 2002, 10:00am (local time), offerors must contact SSgt Gomez to schedule for the site visit. Quotations are due to 355 Contracting Squadron, 3180 S.
