- Description
- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide routine eye examinations, including glaucoma testing, and, if necessary, including dilation, by licensed optometrists, for patients of VA Outpatient Clinics in Appleton and Cleveland WI. The contractor must have two locations at which it will perform the exams: one in Brown County WI and one in Outagamie County WI. The estimated quantity of work is 849 eye exams. It is mandatory that all exams be completed by September 30, 2002. Companies must be able to demonstrate, in their quotations, that they will be able to complete all exams within that time frame. Electronic version and hard copy of solicitation will be available on or about August 2, 2002. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
