Description: NA Personal services contract for Otolaryngology (ENT) Services at Ireland Army Community Hospital, Fort Knox, Kentucky, during the period of 1 November 2002 through 30 September 2003, with four one-year option periods. This acquisition is unrestricted and a ll responsible sources may submit an offer which will be considered. Requests for bid packages must be submitted in writing and may be faxed to 502-624-5869 or 502-624-7165. Telephone requests will not be accepted. Bid packages will not be faxed. Faxed bids will not be accepted. The solicitation will be issued on or about 7 August 2002 with bids due on or about 1 October 2002.
