- Description
- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Opiate Substitution Services for patients of the Milwaukee WI VA Medical center. The place of treatment for VA patients must be located within Milwaukee County WI or a WI county immediately adjacent to Milwaukee County. It is estimated that there will initially be 8 patients per month to be treated. Over the first year of the contract this may increase to 30 patients per month but then is anticipated to level off at 30. This is a firm fixed price contract. Period of performance is from approximately September 1, 2002, August 31, 2003, plus four, one-year optional renewal periods. Electronic version and hard copy of solicitation will be available on or about July 16, 2002. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
