- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this announcement. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. In accordance with FAR 13.003 (b)(1), this solicitation is set aside for small business. NAICS code for this solicitation is 3391. The SIC code is 3842. FAR 19.102 identifies small business as a business with less than 500 employees. Solicitation Number 502-46-02 is issued for a vendor to provide the Department of Veterans Affairs Medical Center, Alexandria, Louisiana, a complete orthotic fabrication system. The system is to consist of a contact digitizer scanner, laptop computer, floor stand for the contact digitizer scanner, CAD/CAM precision milling unit, mill stand, 15 inch color monitor, keyboard, track-ball for the milling unit, system training for station personnel. What we need the system to do is capture the foot revealing contours of light, semi, or full weight bearing positions. Place appliances under the patient?s foot while scanning and correcting for any abnormalities. System needs to be able to introduce software corrections to the contour prior to actual fabrication. The CAD/CAM unit will allow viewer to see computed images of the completed orthotic on the screen prior to milling/fabrication. The system needs to be warranted for 1 full year with an option year to extend. It is suggested, interested offerors contact the medical center for any additional information needed in order to become fully aware of the scope of requirements. Failure to understand any part of this solicitation, shall not relieve the successful offeror from providing the system needed at the agreed on price. Any software license agreement must be reviewed by VA general counsel to assure it conforms to federal law. Payment will be made when equipment is shipped and the necessary training completed by sending an invoice to: VA-FMS-502, PO Box 149971, Austin, TX 78714. The following FAR and VAAR provisions or clauses are incorporated into this synopsis/solicitation: FAR 52.52-2 states: Clause incorporated by Reference (Feb 1998): ?This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.? It further states: ?The full text of a clause may be accessed electronically at this/these internet address(es): www.far.npr.gov, www.access.gpo.gov, www.arnet.gov/far; and www.va.gov/oa&mm/vaar. Applicable clauses and provisions include: 52.212-4, Contract Terms and Conditions?Commercial Items (February, 2002), with addendum; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, (May, 2002). Incorporated under this clause are: 52.203-6; 52.219-23; 52.222-21; 52.222-26; 52.222-35; 52.222-36; 52.222-37; 52.225-13; 52.232-34; 52.222-41; 52.222-42; 52.222-43; 852.237-70; 852.270-4; 852.271-70. Clauses also include 52.212-1 Instructions to Offerors?Commercial Items (October, 2000); Offeror must provide the contracting officer with proof of past performance by submitting three preferable government references with phone numbers and points of contacts. 52.212-3 Offeror Representations and Certifications?Commercial Items (April, 2002), which must be completed and returned to the Contracting Officer with offer.
