- 1. THIS REPLACES CANCELLED RFQ 600-112-02. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with subparts in the Federal Acquisition regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. 2. Specifications. Based upon this Solicitation No. RFQ 600-112-02, VA VISN 22 Network Business Center intends to negotiate a commercial item (FAR Part 12) contract to purchase, name brand or equivalent, Haag-Streit Slit Lamp Model BQ-900-U, four (4) each, for the Optometry Department at VA San Diego Healthcare System, 3350 La Jolla Village Drive, San Diego, CA 92161. Salient characteristics of the equipment are available from manufacturer's specifications. Standard warranty. FOB destination. Delivery must be made within two weeks from acceptance of offer. Offerors must hold prices firm in its offer for 30 calendar days from the date specified for receipt. THIS SOLICITATION IS A TOTAL SMALL BUSINESS SET-ASIDE. 3. Clauses. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. Incorporated herein by reference are the following Federal Acquisition Regulation (FAR) Clauses: 52.211-6: Brand Name or Equal; 52.212-1: Instructions to Offerors - Commercial Items; 52.212-3: Offeror Representations and Certifications - Commercial Items; 52.212-4: Contract Terms and Conditions - Commercial Items; 52.212-5: Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; 52.215-5: Facsimile Proposals; 52.219-6: Notice of Total Small Business Set-Aside; 52.243-1: Changes - Fixed Price. Incorporated herein by reference are the following VA Acquisition Regulation (VAAR) Clauses: 852.203-70: Ethics in Government; 852.211-70: Service Data Manual; 852.211-71: Guarantee (one year); 852.211-71: Rejected Goods; 852.211-75: Technical Industry Standards; and 852.270-4: Commercial Advertising. Contract clauses cited in this solicitation may be accessed electronically at these addresses: http://www.arnet.gov/far/ and http://www.va.gov/oa&mm/vaar/ 4. Instructions, Conditions and Notices to Offerors - Offeror shall provide (1) specifications for "or equal" equipment offered; and (2) a quotation for offered equipment, in the form of Unit Price and Extended Price for number of units specified in paragraph 2 above. Additionally, the offeror shall complete and return all information in accordance with the format specified in FAR 52.212-1, "Instructions to Offerors - Commercial Items" paragraph "b", and FAR 52.212-3, "Offeror Representations and Certifications - Commercial Items" prior to the time specified in paragraph 5 below in order to be considered for award. Payment: Effective January 1, 1999, payment shall be made by electronic funds transfer (EFT). For implementing EFT payments, please contact your financial institution for assistance in completion of the Payment Information Form - SF 3881 for submission to the VA Finance Center in Austin, Texas upon contract award. 5.
