Description
The Centers for Medicare and Medicaid Services (CMS) intends to award under Simplified Acquisition Procedures, a Sole Source basis, under the Simplified Acquisition Procedure (Dollar Threshold of $100,000) a firm fixed price award to Clinical Tools Inc., 431 W. Franklin Street, Suite 24, Chapel Hill, N.C. 27516. This tool will provide easy understanding to educational information to help families and individuals understand health, treatment, daily living and caregiver issues that need to be considered in decisionmaking. This tool needs to be available to broaden audiences with limited English proficiency beginning with the Hispanic/Latino communities. The English version of CarePlanner includes the following features: (1) It identifies necessary modifications, services, and supports needed to manage the risks in any setting. (2) It allows caseworkers, beneficiaries and treatment professionals to assess actual settings a beneficiary is considering. This second level of assessment identifies quality, health and safety issues not addressed in the first level assessment. (3) It weighs risks to identify those that are most likely to affect the health and safety of the beneficiary. (4) It is based on evidence and ?best practices? in the field of home and community based services for assessing the relative risks of different types of community alternatives and the risks associated with features of a specific type of living arrangement. Combining this information with information about the consumer?s abilities and disabilities, it provides a systematic matching of the consumer?s needs and the supports offered by particular community alternatives. (5) It addresses the needs of physically disabled individuals with like health conditions and abilities similar to seniors. It does not address risks and needs that are unique to people with developmental disabilities, chronic mental health conditions, and children with other special health care needs. And, while the primary reason for developing the tool was to assist state agencies programs, beneficiaries, caregivers, agencies and organizations that counsel seniors, other Federal agencies are using CarePlanner. Also, Clinical Tools, Inc., is the only vendor with the proprietary knowledge of the CarePlanner design, content and database, which is essential to the Spanish version of this tool. The content of CarePlanner also draws heavily upon research conducted by Clinical Tools, Inc., in developing SeniorCareWeb, which is a website developed by Clinical Tools, Inc., with a SBIR grant from the Agency for Health Care Research and Quality.
