Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Part 12. This announcement constitutes the only solicitation; proposals are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number F25600-02-R 0035 is issued as a request for quotations. 55 CONS intends to issue a purchase order for Conducting flow testing of fire hydrants on Offutt, AFB, NE. Statement of Work: - Service Contract; Flow Test Fire Hydrants 1. SCOPE: The required services contract consist of furnishing all labor, equipment, and materials, and performing all operations necessary to inspect and test all fire hydrants on Offutt AFB in accordance with NFPA Standard 25, Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems, Paragraphs 4-3.1, A-4.3.1, 4-3.2, Hydrant flow Test shall be conducted in accordance with NFPA 291, Fire Flow Testing and Marking of Hydrants. 2. MAJOR ITEMS OF WORK to be accomplished under this Statement of Work consist of, but are not limited to, the following: A: Inspect and flow test all fire hydrants on main base and in the Capehart Housing area including all support facilities in accordance with NFPA 25. The contractor shall protect existing landscaping and facilities from water flow through use of diffusers and other means. Document the results and any corrective action that will be required. There are approx 192 fire hydrants on the Main Base and 211 in Capehart Housing b. Perform fire Flow Testing in accordance with NFPA 291, on 75 fire hydrants on Main Base and 60 hydrants in Capehart Housing. Fire hydrants will not be marked according to capacity and will remain their current color. Document the test results on Air Force Form 1027, Water Flow Test Record, rather than the form shown in figure 2-11 of NFPA 291. c. Provide completed inspection reports and AF Form 1027?s to Quality Assurance Evaluator, Utilities, Fire Department, Engineering, in four (4) hard copies, including one redlined copy of G-1 tab of the water distribution system. All other documentation to be submitted electronically (Adobe Acrobat Document) within 10 working days after completion of the test. d. Coordinate all work involving the flow testing and marking of the fire hydrants with the foreman of the utilities shop at (402) 294-7586, who will provide locations and as-built drawings of utilities involved in testing. Maps indicating the fire hydrants to be flow tested will be provided to the contractor. Access to fire hydrants in secure areas will require an escort and must be coordinated in advance with the utilities shop. e. The summary of findings/recommendations are to be reviewed and signed by a licensed engineer, the findings should be based on the observations encountered by the testing and flushing personnel during the flushing and testing process and the document preparation process. 3. RECEIPT OF SERVICES: Invoices will be supported by signed service/delivery tickets proving actual services rendered. Invoices will include the name of contractor, order number, date of service rendered, itemized list of services actually rendered, and total amount due for the period covered. All invoices will be forwarded to 55th CES/CEOEM, 106 Peacekeeper Drive, Ste 2N3, Stop Code 4109, Offutt AFB, NE, 68113-4019. 4. ALL WORK shall be accomplished during normal duty hours (0730 thru 1615 hrs) Monday thru Friday. Testing must be accomplished during the month of April. 5. THE CONTRACTOR shall protect existing utilities and surrounding areas. The contractor, at no cost to the government will repair any damage to government property and to the satisfaction of the contracting officer or designated representative. Most of the fire hydrants in Capehart Housing have been replaced within the last two years. The fire hydrants on the Main Base range in age from sixty years to present, and have not beenexercised on a regular basis. The contractor will use reasonable care in operating all fire hydrants. The contractor will be responsible for repairing and or replacing hydrants that are damaged as a result of improper use or abuse. Any fire hydrants that fail in the course of proper operation will be repaired or replaced by the government. 6. ALL ITEMS ESSENTIAL to the completion of this work and not specifically noted or specified shall be provided by the contractor and approved by the contracting officer or designated representative. END STATEMENT OF WORK------------------ This solicitation is issued as a 100 percent small business set-aside; the NAICS Code is 561210; the NAICS size standard is 5 MILLION; and the SIC Code is 8744. All potential bidders are reminded, in accordance with DFARS 252.204-7004, Required Central Contract Registration, that lack of registration in the CCR database BY AWARD DATE will make a bidder ineligible for award. NO PARTIAL QUOTES WILL BE CONSIDERED. NO PARTIAL AWARDS WILL BE MADE.
