- This is a combined synopsis/solicitation for commercial items issued by the 55th Contracting Squadron, Offutt AFB, NE and prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This solicitation constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: F6HXOY21620100, this solicitation is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-07. This announcement constitutes the only solicitation - proposals are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. The Government intends to issue a sole source purchase order to: SQL Power Tools, Inc. to provide (4 each) Zero Impact SQL Monitor; and (4 each) Annual Maintenance Service for Zero Impact SQL Monitor, Delivery 30 days after award to Offutt AFB NE, FOB destination. All work will be accomplished in accordance with the following NAICS Code: 421430, size standard of 500 employees. Solicitation is set aside 100% small business. Item is manufactured by SQL Power Tools, Inc. No substitutes unless approved by the Government. Please acknowledge all requirements on quotation. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. The provision at FAR 52.212-2, Evaluation?Commercial Items, applies to this acquisition. Evaluation will be based on best value including cost and ability to meet requirements. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certification--Commercial Items, with its offer. The clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items (which includes: 52.222-3, Convict Labor (E.O. 11755), 52.222-26, Equal Opportunity (E.O. 11246); 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans (38 U.S.C. 4212); 52.222-36, Affirmative Action for Handicapped Workers (29 U.S.C. 793); 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era (38 U.S.C. 4212); 52.222-41, 52.233-3, Protest After Award (31USC 3553), 52.219-8, Utilization of Small Business Concerns (15 U.S.C. 637 (d)(2) and (3), applies to this acquisition.
