- This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) No. F63336. These solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code for this acquisition is 8331 and the North American Classification System (NAICS) Code is 541611. The FDA intends to issue a purchase order on a sole source basis to Lawrence Carroll, Ph.D., Behavioral Health Associates for the following service: Improvement and Development of Management Systems for CFSAN. Scope of work is to include: The contractor shall work closely with CFSAN staff and other appropriate FDA staff to develop and implement new or revised management procedures and processes as required by the Center: (1) Collect, review and analyze pertinent data on CFSAN system and propose new changes; (2) Analyze and evaluate existing management programs and systems and proposed new changes; (3) Establish timeframes and flow charts for implementing any proposed changes within a timely fashion; (4) Work with CFSAN staff to convey the requirement for implementing any new changes and procedures to the staff (5) Assist in enhancing management/employee relations. Deliverables: the contractor shall submit the following reports: (1) Three copies of monthly written progress reports which clearly indicate all activities and time spent on each, accomplishment to date, and problems encountered with actions taken to rectify them or propose solutions for solving them; (2) Three copies of Final reports shall be a compilation of activities under the contract, any pertinent data collected throughout the project, and necessary backup material. The period of performance will be from date of award through one year. The following FAR Clauses shall apply to this solicitation: 52.212-1 Instructions to Offerors-Commercial; 52.212-4 Contract Terms and Conditions-Commercial Items; and 52.232-33 Electronic Funds Transfers. All responsible sources that can provide and meet the above requirements shall submit written quotations by the due date referenced above. The award is being issued on a sole source basis because Lawrence Carroll, Ph.D. has served as a consultant under CFSAN since January, 1998 and has unique qualifications due to his current knowledge and understanding of CFSAN organizational procedures as well as his long standing knowledge of Center issues. This existing knowledge will result in an overall savings in resources, both fiscal and manpower. FAR provisions can be found at the following website: http://www/arnet.gov.
