Description
The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide for the delivery and management of preventive Primary Medical, Mental Health Care (optional) and the continuity of that care for all Department of Veteran Affairs patients enrolled in the Kenosha, WI Clinic. There may be more than one access point for the clinic contract but all provisions of the contract apply to each site. The clinic must be located within the following delineated boundaries of Kenosha County: East Boundary - Lake Michigan, West Boundary - I-94, North Boundary - Washington Road (hwy 9) and South Boundary - Hwy 165. The contractor will provide health care providers, medical facilities, medical equipment and supplies, and all administrative functions to fulfill the level of care and range of services necessary to meet the primary medical and mental health (optional) needs of the enrolled patients. Short-term pharmaceutical prescriptions will be provided thru another VA contract. Long-term pharmaceutical prescriptions will be provided thru the VA Mail Out Program. The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered there under. This includes responsibility for all contractor employees, subcontracts, agents, and anyone acting for or on behalf of the contractor. This contract will be for a period of one year (from date of award) with the possibilities of 4 (four) one-year options (exercised at the governments discretion). Period of performance is estimated from September 1, 2002 through August 31, 2003, plus four (4) option years. Electronic version and hard copy of solicitation will be available on or about July 3, 2002. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
