- Description
- Home Infusion Services to be furnished by the Contractor to eligible beneficiaries of the VA Medical Center, 650 E. Indian School Road, Phoenix, Arizona 85012-1892 for the period 1 October 2002 through 30 September 2003 with the provision of four (4) one-year options. If all 4 Options are exercised by the Government the contract will extend through 30 Septemebr 2007. Home Infusion Services include Antimicrobial Infusion Therapy, Total Parenteral Nutrition Therapy, Pain Management, Chemotherapy, and Hydration Therapy. Contractor must be licensed by the State of Arizona as a Home Health Care Agency and be JCAHO accredited. All responsible sources may submit a quotation which shall be considered by the VA. Solicitation will be made available electronically through the following websites: http://www.fedbizopps.gov and http://www.bos.oammm.va.gov
