- NA The National Transportation Safety Board (NTSB) intends to issue a task order against a GSA schedule contract to develop a Web-based data collection system to acquire information from pilots involved in aviation accidents. This information is currently collected on an 11-page hard-copy form (NTSB Form 6120.1 approved by OMB No. 3147-0001). Pilots involved in an accident are required to provide the NTSB with information concerning the event, aircraft, aircraft owner, aircraft operator, pilot(s), airport, and weather. The content of the data form is well defined and the NTSB will continue to accept data submitted on the hard-copy form. The purposes of this contract are to develop an electronic version of Form 6120.1 that will capture information submitted by the pilot and allow these data to be electronic -- click under Additional Information to view complete announcement text
