- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format used in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a writte solicitation will not be issued. DAHA25-02-T-0021 is being issued as a Request for Quotations (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-07. This combined synopsis/solicitation has been set-aside 100& for small businesses. The SIC Code is 3572. CLIN 0001: WYSE 3360SE Winterm, 32MB Ram, Keyboard and Mouse (Name brand only and new). Quantity of 30 each. CLIN 0002: NEC AS70 17 inch monitors (brand nam e only and new) Quantity of 30 each. Delivery to Lincoln, Nebraska, DCSIM, Attention: Bob Kerns, 1237 Military Road, Lincoln, NE 68508. Delivery no later than June 28, 2002. Any and all shipping costs must be included in the pricing of each item and not as a separate charge. Any vendor quotes received with separate shipping charges will be considered non-responsive. The following FARS are application to this combined synopsis/solicitation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Eval uation-Commercial Items (lowest pricing on brand name); 52.212-3, Offeror Representations and Certifications-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Needed to Implement Statutes on Executive Orders-Commercial Items; and 52.247-34, FOB-Destination. All quotes must be sent by e-mail to sandra.leach@ne.ngb.army.mil no later than 4:00 p.m. CDT, Thursday, June 6, 2002. All quotes must have the following mandatory information to be consid ered responsive: the RFQ Number, Contractor Name and Address, Contractor Telephone and Fax Number, Contractor Point of Contact, and Contractor Duns & Bradstreet Number. Failure to include any of this mandatory information will make vendor quote non-respons ive. All vendors/prospective contractors must be registered in the CCR Database (www.ccr.gov) prior to award on Friday, June 7, 2002. If not active or registered by 7:30 a.m. CDT on Friday, June 2, 2002, vendor quote will be considered non-responsive.
