- Description
- Contractor shall provide all necessary labor, supervision, materials, supplies and equipment necessary for the inscription of granite niche covers at Minnesota State Veterans Cemetery, 15550 Highway 115, Little Falls, MN 56345. Contract covers base period October 01, 2002 through September 30, 2003, with four 1-year options to extend the term of the contract through September 30, 2007. Inscription requirements will be released to, and performed by, the contractor on a biweekly basis. Inscription work will not be performed at the cemetery. Contractor will be responsible for: pick up; packaging sufficient to prevent damage during shipment and handling; inscribing; and returning niche covers to cemetery. This acquisition is 100 percent set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 561990 and a size standard of $6 M are applicable. All responsible small business sources may submit a quote. Request for a hard copy of the solicitation must be in writing to Mr. Mark Sanderson, contract Specialist, and accompanied by a check or money order in the amount of Five Dollars ($5.00) (non-refundable), payable to "U.S. Department of Veterans Affairs." Please reference Solicitation Number RFQ 786-68-02 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about June 25, 2002. The solicitation may also be obtained FREE OF CHARGE, from VA's Business Opportunities System via the internet at the following URL address on or about June 25, 2002: http://www.bos.oamm.va.gov/solicitation?number=786-68-02. Any amendments to the solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. See Commerce Business Daily Numbered Note 1.
