- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format used in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. DAHA25-02-T-0019 is being issued as a request for quotations (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acqusition Circular 01-07. This combined synopsis/solicitation has been set-aside 100% for small businesses. The SIC Code of 3572. 56.6K ISA Internal Fax/Modem that cannot require windows to operate, must be hardware settable, must run under DOS 6.22, must be compatible and include driv ers for Windows 2000 and NT 4.0. Mandatory that vendor provide make, model, and part number. Quotes will be considered non-responsive if make, model, and part number is not on vendor quote. Quantity of 500 each. For all technical information regarding th e specifications on the fax/modem, vendor should call 402-309-1920. DO NOT CALL Sandy Leach for technical questions. Any and all shipping costs must be included in the price of each fax/modem and not as a separate charge. Vendor quotes will be considered non-responsive if they have a separate charge for shipping. The followings FARS are applicable to this combined synopsis/solicitation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (lowest price that meets all specifications); 52.212-3 Offeror Representations & Certifications-Commercial Items; 52.212-4, Contract Terms & Conditions-Commercial Items; 52.212-5, Contract Terms & Conditions Needed to Implement Statutes on Executive Orders-Commercial Items; 52.247-34, FOB-Destination. You may use most any type of format (word, excel, PDF, regular e-mail) to send your quote, however it must be by e-mail only. The following information is mandatory and must be included on your quote: Vendor Name & Address, Vendor teleph one and fax number, Vendor Point of Contact for quote, Vendor Duns & Bradstreet Number. Failure to provide any or all of this information will make vendor quote non-responsive. You may also wish to include your e-mail address. All vendors/prospective co ntractors must be registered in the CCR Database (www.ccr.gov) prior to award date of June 6, 2002. If not active or registered by 7:30 a.m. CDT, Thursday, June 6, 2002, vendor quote will be considered non-responsive. All quotes (by e-mail only) must be e-mailed no later than 4:00 p.m. CDT on Wednesday, June 5, 2002. No other method of sending a quote will be accepted.
