Description

NA Madigan Army Medical Center, Tacoma, WA intends to establish a firm fixed price contract that will provide for the services of Refractive Eye Laser Surgery with an established eye care facility within the local commuting distance of Madigan. The establishe d eye facility shall be able to provide the use of its facility, an FDA approved excimer laser capable of correcting Hyperopia, Myopia and Myopic Astigmatism, for use in PRK and LASIK surgical procedures, the following personnel, preoperative ophthalmic te chnician, Keratome technician, and secretary/receptionist. The request for proposal number is DADA13-02-R-0002. The NAICS code is 621320 and the size standard is $6.0 million. Estimated date of release is May 27, 2002 and the anticipated closing date is June 14, 2002. All responsible sources may submit an offer, which shall be considered.
