- Description
- This is a combined synopsis/solicitation issued for commercial items and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items; FAR Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. Simplified Aquisition Procedures apply and VA will conduct the procurement according to a combination of parts 12 and 13. This announcement for Request For Quotation RFQ #689-69-02 entitled Cardiac Stents is hereby issued and incorporates provisions and clauses in effect through FAR FAC 2001-07 and VAAR 97-8. NAICS code 339113 applies for this procurement and the small business size standard is 500 employees. This BOS announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractors quotes are hereby requested for the following: 1) S660 Over the Wire (OTW) 2.75 9mm stent-2 each; 2) S660 OTW 2.75 12mm stent-2 each; 3) S660 OTW 2.75 15mm stent-2 each; 4) S660 OTW 2.75 18mm stent-2 each; 5) S660 OTW 2.75 24mm stent-2 each; 6) S7 OTW 3.0 9mm stent-5 each; 7) S7 OTW 3.0 12mm stent-5 each; 8) S7 OTW 3.0 15mm stent-5 each; 9) S7 OTW 3.5 15mm stent-5 each; 10) S7 OTW 4.0 15mm stent-5 each. Delivery shall be FOB Destination. The following provisions and clauses shall apply to this solicitation: FAR 52.252-2 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
