Description
The Department of Veterans Affairs (VA) Vocational Rehabilitation and Employment Service (VR&E), Chardon Ave., San Juan, Puerto Rico has a requirement to provide Vocational Rehabilitation and Employment Service (VR&E) in Initial Evaluation Chapter 31. The intent of this contract is for the Contractor to furnish to the Veterans Administration Regional Office (herein referred to VARO), VA Medical Center (herein after referred to as VAMC and/or VA), Initial Evaluation Services, as described in Title 38, CFR 21.50 and 21.52 to eligible veterans, and service members and in accordance with all the terms, conditions and provisions of this contract. Counseling service for purposes of this contract is counseling provided by a clinical psychologist. The primary purpose of Initial Evaluation Services through the use of established counseling and assessment technique to gather the information needed to allow an appropriate Vocational Rehabilitation and Counseling Division staff person to make determinations regarding entitlement of assistance under the Chapter 31 and Chapter 15 programs. The Contractor (counselor) is not allowed to discuss the implications of their findings as they relate to the individuals potential entitlement to services. Initial Evaluation Services as described are available to individuals that have been determined to have basic eligibility and have been authorized those services by an appropriate Vocational Rehabilitation and Counseling Division (hereinafter referred to as VR&E) staff person. 1)Eligible veterans, and service members have applied under 38 CFR and have been authorized these services through the VR&E Division?s program (28) at VA Regional Office, Chardon Ave, Federal Office Building, Room 315, Hato Rey Puerto Rico. 2)The Contractor shall provide the following Initial Evaluation Services: a)Assessment of the handicapping effects of the veteran?s service-connected and non-service-connected disabilities on employability and independence in daily living; b)Assessment of the veteran?s residual physical and mental capabilities which contribute to employability and independence in daily living; c)Assessment of the veterans ability to function independently in family and community; d)Assessment of prior program participants will also include assessment of transferable jobs skills analysis and marketability of those skills; e)Assessments using authorized instruments (aptitude, abilities, and interest, etc.) to provide additional information; f)Gathering of the veteran?s personal history including: 1.Educational and Training; 2.Employment; 3.Non-service-connected disability(ies), and 4.Family and community adjustments g)Identification of needs and services to meet those needs (Functional Goals);h)Discussion and identification of career goals with the Veteran and/or service-member. (The Contractor shall not establish a final rehabilitation goal). It is the responsibility of the U.S. Department of Veterans Affairs (VA) to reach agreement with the counselee on the rehabilitation goal; and i)Competed reports to the VA, at such a time and in such a manner as shall be prescribed by the VA, containing analysis, conclusions, and recommendations based upon the Initial Evaluation findings. j)Discussion of findings and report with counselee. The Initial Evaluation shall develop such information as is necessary to assist the VA to make appropriate decision on entitlement and development of an appropriate Rehabilitation Plan. Initial Evaluation for ?Special? Chapter 31 Cases: These are Chapter 31 cases that have been an Initial Evaluation in the past and only specific aspects of the case needs to be updated. The VA shall have reviewed the case and shall have specified specific issues that are to be addressed in the evaluation. This is short-period contract (requirement contract type)to have the services coverage until a national contract be awarded and implemented. The contract includes a base period of approximately 4 month with a Government right to extend the services for additional period not to exceed 6 month.
