- This synopsis is hereby modified to ADD a DRAFT Statement of Work (This document will be available at http://www.rdc.noaa.gov/~amd/SOLINDEX.HTML on May 6, 2002). The synopsis is further modified to ADD two custom profile codes: 201 - Resource Mapping and 202 - Benthic Habitat Mapping. And, the synopsis is also modified to change the format/submission requirements as follows. FROM: Item 5 of the SF 255 must be completed in full for the venture to be considered. Only one (1) SF 255 shall be submitted for joint ventures. USE STANDARD PAGES OF THE SF 254 and 255 (Rev. 11/92) prescribed. Offerors shall submit 3 (three) copies of the SF 254(s) and 255(s) for each package, in order to be considered. Each of the 3 (three) package copies will be submitted in a three-ringed binder with sections indicated by labeled tabs, will have the firms name on the cover, and will not exceed 30 (thirty) pages (double-sided) in length (including all attachments). The original copy will be marked "Original" on the cover. No more than 10 (ten) brief resumes shall be submitted as part of item 7 of the SF 255; each resume shall not exceed one page single-sided in length using standard 10-point type size. Item 8 of the SF-255 should be limited to 1 (one) page per example project for a total of no more than 5 (five) projects. Item 10 of the SF 255 shall be limited to 5 (five) single-sided pages, also using standard 10-point type size. All other material such as brochures, samples of work, attachments or extra pages will not be considered and will be discarded upon receipt. If a firm has multiple offices, for each of the criteria requiring demonstrated abilities and experience, show project experience for each specific office and list the personnel in the office who would be assigned to perform the work. TO: Item 5 of the SF 255 must be completed in full for the venture to be considered. Only one (1) SF 255 shall be submitted for joint ventures. USE STANDARD PAGES OF THE SF 254 and 255 (Rev. 11/92) prescribed. Offerors shall submit one original plus 3 (three) copies of the SF 254(s) and 255(s) for each package in order to be considered. All copies shall be submitted in a three-ringed binder and shall not exceed 30 (thirty) double-sided pages or 60 pages in total (including all attachments). The original copy will be marked "Original" on the cover. Item 8 of the SF-255 shall identify a minimum of five (5) projects. ALL OTHER MATERIAL SUCH AS BROCHURES, SAMPLES OF WORK, ATTACHMENTS OR EXTRA PAGES SHOULD NOT BE SUBMITTED AS IT WILL NOT BE CONSIDERED FOR EVALUATION.
