Description

NA Madigan Army Medical Center, Tacoma, WA intends to establish a firm fixed price contract for a base period plus four (4) one year option periods that will provide for maintenance service and repairs to the Computed Radiography Digital Dry Laser Film Proces sor System manufactured by Fuji Medical Imaging. The system consists of 3 each FCR5000 w/IDT741; 4 each FM-DP2636; 3 each FM-DP3543; 2 each DASM; 1 each MF-300S; and 1 each QA 771 WS. Offeror shall be responsible for providing qualified technicians to perf orm preventive and corrective maintenance, calibrations, unlimited on-call service for equipment malfunctions, replacement parts, loaner equipment when the system will be down for more than two (2) working days, software upgrades, as applicable, and itemiz ed service reports. The Request for Proposal number is DADA13-02-R-0004. The NAICS code is 811219 and the size standard is $6.0 million. The proposal can be downloaded from FedBizOpps website: www.eps.gov. Telephonic requests for copies of the proposal wi ll not be honored. Estimated date of release is April 19, 2002 and the anticipated closing date is April 29, 2002. All responsible sources may submit an offer, which shall be considered.
