- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. DAHA25-02-T-0012 is being issued as a Request for Quotations (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-06. This soli citation has been set-aside 100% for small businesses. The SIC Code is 3571. Please read the following specifications carefully. 56.6K ISA Internal Fax Modem. CANNOT REQUIRE WINDOWS TO OPERATE. Must be hardware settable and must run under Dos 6.22. Must also be compatible and include drivers for Windows 2000 and NT 4.0. Quantity of 500 each. Do not sent a quote unless this fax/modem meets these exact specifications. You must provide me with the name brand, model number and part number on your quote. F ailure to submit this information will make your quote non-responsive. All questions concerning the modems should be addressed to the following e-mail addresses: michael.rudebusch@ne.ngb.army or david.suing@ne.ngb.army.mil. Do not contact Sandy Leach for any type of technical question concerning the modem. Delivery to Lincoln, Nebraska, CRRA, 2960 North Park Road, Lincoln, Nebraska 68524-2402. Delivery 30 days after receipt of purchase order. Shipping costs must be included in the pricing of each item and not as a separate charge. If your quote shows an extra charge for shipping your quote will be considered as non-responsive. The following FARS are applicable to this combined synopsis/solici tation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (must meet all specifications and lowest price); 52.212-3, Offeror Representations & Certifications-Commercial Items; 52.212-4, Contract Terms & Conditions-C ommercial Items; 52.212-5, Contact Terms & Conditions Required to Implement Statutes on Executive Orders-Commercial Items; 52.247-34, Destination Shipping. You may use any format to send the quote however this following information is mandatory on this quo te along with the requested item information: Solicitation Number, Name and Address of Vendor, Vendor phone and fax number, Point of Contact, and Duns & Bradstreet Number.
