Description
REAGENTS FOR TOXICOLOGY TEST PROCEDURES TO BE COMPATIBLE FOR USE ON SYVA 30R BIOCHEMICAL SYSTEM. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Part?s 2, 12, and 13, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 546-36-02. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-05. The NAICS code for this solicitation is 325413. The small business size standard is 500 employees. This acquisition is unrestricted. Furnish and deliver the following reagents for drug testing procedures for use on and must be compatible with the Syva 30R Biochemical System. Reagents and Lot Numbers must be consistent throughout the fiscal year. The SCHEDULE OF PRICES shall be listed and priced as follows: Item 1. Reagent for Opiate Test Procedure, 100mL, Emit II (9B109), estimated quantity 25 each per year, $________ ea., Total $__________. Item 2. Reagents for Mono Amphetamine Test Procedure, 100mL, Emit II (9C019), estimated quantity 25 each per year, $__________ ea, Total $___________. Item 3. Reagents for Barbiturate Test Procedure, 100mL, Emit II (9D019), estimated quantity 25 each per year, $__________, Total $_______________. Item 4. Reagents for Methadone Test Procedure, 100mL, Emit II (9E019), estimated quantity 25 each per year, $____________, Total $_______________. Item 5. Reagent for Benzodiazepine Test Procedure, 100mL, Emit II, (9F019), estimated quantity 14 each per year, $____________, Total $_______________. Item 6. Reagent For Cocaine Test Procedure, 100 mL, Emit II, (9019), estimated quantity 25 each per year, $__________, Total $______________. Item 7. Reagent for Cannabinoid Test Procedure, 20ng, 100ml, Emit II (9M619), estimated quantity 25 each per year, $_____________, Total $____________. Kits will be ordered by the VA on an as needed basis. Contract period will be from date of award through September 30, 2002 with 1 one-year option. Deliver FOB Destination to Special Chemistry, NH Room #425, VA Medical Center, 1201 NW 16th Street, Miami, Florida 33125. Purchase Orders will be placed against this contract on an as needed basis. Delivery is to be made within 7-10 days of order placement. Items needing refrigeration are to be marked on the outside of delivery box. This is a firm fixed price contract.
