- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. DAHA25-02-T-0008 is being issued as a Request for Quotations (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circliat 01-06. This so licitation has been set-aside 100% for small businesses. The SIC Code is 3571. 10/100 PIC Ethernet Card with RJ45 Connection and must be Windows 2000 and NT 4.0 Compatible. Quantity of 400 each. Delivery to Lincoln, Nebraska, Computer Rebuild and Repai r, 2960 North Park Road, Lincoln, NE 68524-2402. Delivery 30 days after receipt of purchase order. Shipping cost must be included in the pricing of each card and not as a separate charge. If pricing is sent with shipping as an extra charge, your quote will be deemed as non-responsive. The following FARS are applicable to this combined synopsis/so licitation: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (meets all specifications and lowest price; 52.212-3, Offeror Representations & Certifications-Commercial Items; 52.212-4, Contract Terms & Conditions-Co mmercial Items; 52.212-5, Contract Terms & Conditions Required to Implement Statutes on Executive Orders-Commercial Items; 52.247-34, Destination Shipping. You may use any format even just a regular e-mail message to send your quote, however, the followin g information is mandatory and must be included in your quote: solicitation number, vendor name and address, phone and fax number, point of contact, Duns and Bradstreet number. If your quote is missing any of this information it will be deemed as non-resp onsive. Your e-mail address is not required, however, vendor may also want to include it in their quotation. All vendors responding must be registered with the Central Contractor's Registration (www.ccr.gov) and must be have an active status.
