- NA This is a combined synopsis/solicitation for commercial items in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requ ested and a written solicitation will not be issued. DAHA25-02-T-0009 is being issued as a Request for Quotations (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-06. This solicitatio n has been set-aside 100% for small businesses. The SIC Code is 3571. 17in, .27mm, SVGA Monitors, 1280x1024 Resolution, Energy Star Compliant. Quantity of 400 each. Delivery to Lincoln, Nebraska, Computer Rebuild and Repair, 2960 North Park Road, Lincoln , Nebraska 68524-2402. Delivery 30 days after receipt of purchase order. Telephone number for point of contact on all technical information is 402-309-1920 extensions 2545,2546, and 2541. Do not call Sandy Leach for technical information. Shipping costs must be included in your pricing of the monitors and not as a separate charge. If the quote is received with shipping as a separate charge, vendor quote will be deemed as non-responsive. The following FARS are applicable to the combined synopsis/solicita tion: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items (meets specifications and lowest price); 52.212-3, Offeror Representations & Certifications-Commercial Items; 52.212.4, Contract Terms & Conditions-Commercial Items; Contract Terms & Conditions Required to Implement Statutes on Executive Orders-Commercial Items; 52.247-34, Destination Shipping. You may use any format, even a regular e-mail message to send your quote, however the following information is mandato ry: solicitation number, vendor name and address, telephone and fax number, point of contact, and Duns and Bradstreet number.
