See Upload 1-1 Cover letter and Solicitation. NAICS: 541690, Numbered/Non-Numberes Note(s): Do Not Apply and Order of Magnitude Threshold: (Estimated) Under $10M. The purpose of this amendment is to extend the closing date for the submission of proposals to May 3, 2002 and to incorporate the changes into the solicitation documents and to make reference to the questions and anwers detailed below. RFP 492-02-006, Amendment No. 001 ?Strengthening Private Sector Diagnosis and Treatment for TB Control? Changes to the RFP: 1. On the RFP Cover Letter and SF 33, replace the closing date for submission of proposals to May 3, 2002 at 3:00 pm local time. 2. On Section H, Special Contract Requirements: 2.a. Replace H.6(a)(1), Personnel Compensation, Limitations with the following: ?(1) Salaries and wages may not exceed the Contractor?s established policy and practice, including the Contractor?s established pay scale for equivalent classifications of employees.? 2.b.Incorporate the following as H.9: ?H.9. Implementation of E.O. 13224 ? Executive Order on Terrorist Financing. The selected contractor is reminded that US Executive Orders and US law prohibits transactions with, and the provision of resources and support to, individuals and organizations associated with terrorism. It is the legal responsibility of the selected contractor to ensure compliance with these Executive Orders and laws. This provision must be included in all subcontracts/subawards issued under this contract.? 3. On Section I.3(c), Option to Extend, revise as follows: ?(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 4 years.? 4. On Section L.7(a)(iii), General Instructions to Offerors, replace the last line with the following: ?(iii) ?.. Proposals should be properly marked and must be received at the Office of Regional Procurement, USAID/Philippines, 8/F PNB Financial Center, Roxas Blvd., Pasay City, 1308, Philippines by 3:00 pm, Manila Time, on May 3, 2002 to be considered (FAR 15.208(b)).? 5. Replace ?Section L.7?, Instructions for the Preparation of the Technical Proposal, with ?Section L.8?, and subsequent numberings thereafter. 6. On Section L.8(d), Instructions for the Preparation of the Technical Proposal, revise the sequential numbering from 1 to 4. 7. On Section L.8(d)(3), Project Staff and Qualification, replace (iii) as follows: ?iii. The offeror shall indicate the specialty and assignment of personnel assigned according to Prime Contractor and subcontactor(s), both long and short term, for discreet work components and length of time in person-months based upon the activities in the work plan. In addition to the detailed CVs above, a one to two page graphic presentation of the skills areas of proposed long-term and short term consultants shall also be provided. In a table format, proposed consultants (both long-term and short-term), should be listed accordingly and by appropriate skills area across the top.? QUESTIONS AND ANSWERS: Q.1. Closing Date for submission of proposals. A.1. April 22, 2002 at 3:00pm, Manila Time Q.2. Is this project currently on-going and is there currently an incumbent contractor? A.2. Please be advised that the subject RFP is for the implementation of new project activities. Q.3.1. The RFP does not refer to subgranting at all however it does refer to ?coordination with other contractors and grantees under the PHN strategic objective.? Please clarify if subgranting will be allowed under this RFP? If yes, please advise if there is a maximum funding level for subgrants issued by the prime contractor. Q.3.2. If an applicant is a consortium of organizations, is it allowable for the prime contractor to have both subcontractors and subgrantees, or must all relationships be via a contract mechanism? A.3. The Mission has a roster of partners, both contractors and grantees, in the implementation of its population, health and nutrition (PHN) initiatives. With the implementation of this new activity on TB, the selected contractor will be expected to coordinate/liaise with the other partners. In reference to ?subgranting? or ?subcontracting? under the project, it is left to the proponents/offerors to determine the appropriate approach to their proposed strategy in implementing the project. Q.4. Is there a page limitation for the technical proposal? A.4. There is no page limitation for the technical proposal. Q.5. If an applicant is a consortium of organizations, is each subcontractor required to submit all the items (a-k) under Section L.10, Instructions for the Preparation of Cost Proposal? Only items e, f, g specifically state that subcontractors must provide these items. Does this mean that the other items are required only from the Prime applicant? A.5. The cost proposal shall include required information from subcontractors only for items ?e.?, ?f?, and ?g?, of Section L.10. On the otherhand, the Prime offeror shall refer to all other items and respond to each accordingly and as appropriate. Q.6. Must the SF-LLL be submitted as part of the business proposal? It is included as an attachment to the RFP, but is not listed as a requirement item under Section L.10, Instructions for the preparation of Cost proposal. A.6. Yes, SF-LLL, Disclosure of Lobbying activities shall be submitted as part of the business proposal.
